小骨窗微侵袭手术与开颅手术治疗幕上自发性脑出血的效果及安全性比较

摘　　要：目的比较小骨窗微侵袭手术与开颅手术治疗幕上自发性脑出血的效果及安全性。方法将100例幕上自发性脑出血患者根据治疗方法的不同分为对照组(50例,开颅手术)与试验组(50例,小骨窗微侵袭手术)。比较两组的临床效果。结果试验组患者GOS评分为2分的比例低于对照组,GOS评分为5分的比例高于对照组(P<0.05)。试验组的手术用时、手术出血量及住院时间均优于对照组(P<0.05)。治疗3 d后,两组的NIHSS评分均降低,且试验组低于对照组(P<0.05)。试验组的并发症总发生率及再出血率均低于对照组(P<0.05)。结论小骨窗微侵袭手术与开颅手术治疗幕上自发性脑出血患者均能够获得一定的效果,但前者的应用价值更高。Objective To compare the efficacy and safety between minimally invasive small bone window craniotomy and craniotomy in the treatment of supratentorial spontaneous intracerebral hemorrhage.Methods A total of 100 patients with supratentorial spontaneous intracerebral hemorrhage were divided into control group(50 cases,craniotomy)and experimental group(50 cases,minimally invasive small bone window craniotomy)by different treatment methods.The clinical effects of the two groups were compared.Results The proportion of patients with GOS score of 2 in the experimental group was lower than that in the control group,and the proportion of patients with GOS score of 5 was higher than that in the control group(P<0.05).The operation time,the amount of operation bleeding and hospitalization time in the experimental group were better than those in the control group(P<0.05).After 3 days of treatment,the NIHSS scores in the two groups decreased,and that in the experimental group was lower than the control group(P<0.05).The total incidence of complications and rebleeding rate in the experimental group were lower than those in the control group(P<0.05).Conclusion Minimally invasive small bone window craniotomy and craniotomy in the treatment of supratentorial spontaneous intracerebral hemorrhage can achieve certain results,but the former has higher application value.
