彩色多普勒超声监测早孕期绒毛滋养动脉预测先兆流产的价值分析

摘　　要：目的:探讨彩色多普勒超声监测早孕期绒毛滋养动脉预测先兆流产的价值。方法:选择2018年3月~2019年3月于某院就诊治疗的60例发生先兆流产的早孕期(孕5~8周)孕妇为研究对象,根据保胎治疗结局分为妊娠组和流产组,同时选择同时期30例正常宫内妊娠的孕妇为对照组。就诊时对所有孕妇进行人绒毛膜促性腺激素(β-hCG)和血清孕酮(P)的检测,同时采用阴道彩色多普勒超声对绒毛滋养动脉的血流指标进行检查,比较各组滋养动脉PSV、EDV、S/D值、RI值以及三维超声血流参数VI、FI、VFI。结果:3组孕妇血清中β-hCG和P的含量水平由低到高依次为流产组、妊娠组、正常组,组间两两比较具有统计学差异(P<0.05);3组孕妇EDV值由低到高依次为流产组、正常组、妊娠组,S/D值和RI值由低到高依次为妊娠组、正常组、流产组,组间两两比较具有统计学差异(P<0.05);3组孕妇VI值和VFI值由低到高依次为正常组、妊娠组、流产组,组间两两比较具有统计学差异(P<0.05)。结论:孕妇滋养动脉血流状态与早期先兆流产的预后存在一定的相关性,可以作为保胎治疗的参考依据。Objective:To explore the value of color Doppler ultrasound monitoring the trophoblast artery in early pregnancy to predict threatened abortion.Methods:From March 2018 to March 2019,60 pregnant women with threatened abortion(5~8 weeks of gestation)who were treated in a hospital were selected as the study object.According to the outcome of the treatment,they were divided into pregnancy group and abortion group.Meanwhile,30 pregnant women with normal intrauterine pregnancy in the same period were selected as the control group.All pregnant women were examined for human chorionic gonadotropin(β-hCG)and serum progesterone(P).The blood flow indexes of the trophoblastic arteries were examined by vaginal color Doppler ultrasound.PSV,EDV,S/D,RI and three-dimensional ultrasound blood flow parameters VI,FI and VFI of the trophoblastic arteries were compared.Results:The serum levels ofβ-hCG and P in the three groups were in the order of abortion group,pregnancy group and normal group from low to high,and there was statistical difference among the three groups(P<0.05).The EDV values of the three groups were in the order of abortion group,normal group and pregnancy group from low to high,the S/D values and RI values were in the order of pregnancy group,normal group and abortion group from low to high,and there was statistical difference among the three groups(P<0.05).The values of VI and VFI in three groups were normal group,pregnancy group and abortion group from low to high,and there was statistical difference among the three groups(P<0.05).Conclusion:There is a certain correlation between the flow state of trophoblastic artery and the prognosis of early threatened abortion,which can be used as a reference for the treatment of fetal protection.
