斑块旋切术联合药涂球囊在糖尿病股腘动脉病变治疗中的应用

摘　　要：目的探讨斑块旋切术联合药涂球囊治疗糖尿病股腘动脉病变的短-中期临床疗效。方法回顾性分析2016年4月至2018年7月应用斑块旋切术联合药涂球囊治疗的32例糖尿病股腘动脉病变患者临床资料。收集围术期并发症、术后临床症状及踝肱指数(ABI)改善情况,随访Fontaine分级、靶血管一期通畅率。结果手术均获得成功,围术期并发症发病率为15.62%(5/32),术后临床症状均较术前改善,踝肱指数升高明显。随访8~17个月,平均(12.74±2.63)个月。Fontaine分级较术前有不同程度改善。术后3、6、12个月,ABI较术前明显提高,差异具有统计学意义(P<0.05),靶血管一期通畅率分别为93.75%(30/32)、84.37%(27/32)、68.75%(22/32)。结论斑块旋切术联合药涂球囊治疗糖尿病股腘动脉病变安全有效。Objective To investigate the short and mid-term clinical outcome of directional atherectomy combined with drug-coated balloon in treatment of femoropopliteal artery disease in diabetic patients.Methods From April 2016 to July 2018, The clinical data of 32 diabetic patients with femoropopliteal artery disease undergoing directional atherectomy combined with drug-coated balloon were retrospectively analyzed. The perioperative complications, clinical symptoms, ankle brachial index(ABI), Fontaine classifications, first patency rate of target vessels were observed.Results The procedures were successful in all patients, The perioperative complications were 15.62%(5/32). Postoperative clinical symptoms are improved compared to before surgery. ABI increased significantly. Follow-up for 8-17 months, average(12.74±2.63) months. Fontaine classifications have improved to different degrees than before surgery. 3, 6, 12 months after operation, ABI was significantly higher than before, and the difference was statistically significant(P<0.05),the first patency rate of target vessels were 93.75%(30/32),84.37%(27/32),68.75%(22/32).Conclusion Directional atherectomy combined with drug-coated balloon is a safe and effective treatment strategy in the treatment of femoropopliteal artery disease in diabetic patients.
