滋阴润肺法联合穴位敷贴防治放射性肺炎(气阴两伤证)的临床观察

摘　　要：目的:观察滋阴润肺法联合穴位敷贴防治放射性肺炎(气阴两伤证)的临床效果。方法:将72例放射性肺炎患者随机分为对照组和治疗组,每组36例。对照组予西医常规治疗,治疗组在常规治疗基础上给予中医滋阴润肺法联合穴位敷贴进行治疗,连续治疗14 d。通过观察中医证候积分、生活质量KPS评分变化对比两组治疗的临床效果,监测血清中氧化应激指标丙二醛(malondialdehyde,MDA)、超氧化物歧化酶(superoxide dismutase,SOD)、总抗氧化能力(total antioxidant capacity,T-AOC)水平,评价用药的安全性。结果:治疗组、对照组的有效率分别为91.67%(33/36)、75.00%(27/36),治疗组有效率更高(P<0.05)。组内比较,两组患者治疗后乏力、刺激性干咳、口干、喘息等主要症状的中医证候评分降低,生活质量KPS评分上升,血清氧化应激指标MDA水平降低,SOD、T-AOC水平明显升高,差异均有统计学意义(P<0.05);组间比较,治疗组中医证候评分更低、生活质量KPS评分更高,氧化应激指标变化程度更明显,差异均有统计学意义(P<0.05)。治疗组不良反应发生率为11.11%(4/36),明显低于对照组的25.00%(9/36)(P<0.05)。结论:滋阴润肺法联合穴位敷贴有利于缓解放射性肺炎(气阴两伤证)患者的临床症状,提高其生活质量,改善氧化应激状态,防治肺纤维化的发生,用药安全可靠。Objective:To observe the clinical effect of nourishing Yin and nourishing lung combined with acupoint application in the prevention and treatment of radiation pneumonia(Qi and Yin deficiency syndrome).Methods:Totally 72 patients of radiation pneumonia were randomly divided into control and treatment groups,with 36 cases in each group.The control group was treated with conventional western medicine,and the treatment group was treated with Chinese Medicine nourishing Yin and nourishing lung combined with acupoint application on the basis of conventional treatment,for 14 days.The change of TCM syndrome score and life quality KPS score were observed to compare the clinical effects between the two groups.The levels of serum oxidative stress indicators malondialdehyde(MDA),superoxide dismutase(SOD)and Total antioxidant capacity(T-AOC)were monitored,and the safety of medication was evaluated.Results:The effective rate was 91.67%(33/36)and75.00%(27/36)respectively in treatment and the control groups,and the treatment group showed higher effective rate than control group(P<0.05).The TCM syndrome score of the main symptoms,including fatigue,irritant dry cough,dry mouth and wheezing significantly decreased in two groups after treatment,and the quality of life KPS score significantly increased in two groups;The MDA level of serum oxidative stress index significantly decreased,and the level of SOD and T-AOC significantly increased in two groups,with statistically significant differences(P<0.05).The treatment group showed lower TCM syndrome scores,higher quality of life KPS scores,and more significant changes in oxidative stress indicators(P<0.05).The adverse reaction incidence rate was11.11%(4/36)in the treatment group,which was significantly lower than control group[25.00%(9/36)](P<0.05).Conclusion:The method of nourishing Yin and nourishing lung combined with acupoint application is beneficial to relieve the clinical symptoms of the patients with radioactive pneumonia(both Qi and Yin deficiency syndrome),increase the qualit
