加味大承气汤辅治SAP患者效果及对胰腺微循环、炎症反应的影响

摘　　要：目的探究加味大承气汤辅治重症急性胰腺炎(SAP)患者效果及对胰腺微循环、炎症反应的影响。方法回顾性分析112例SAP临床资料,48例患者为常规治疗,64例患者在常规治疗基础上加用加味大承气汤辅助治疗,比较两组患者疗效、恢复情况、血淀粉酶含量、胰腺微循环指标、炎症指标差异。结果治疗7d后,观察组患者治疗总有效率为92.19%,高于对照组的79.17%,差异有统计学意义(P<0.05);经治疗,观察组患者第一次排气时间、腹痛消失时间、肠鸣音恢复时间、住ICU天数短于对照组,差异有统计学意义(P<0.05);治疗7d后,两组患者血清淀粉酶、胰腺微循环指标[血小板活化因子(PAF)、凝血酶原时间(PT)、D二聚体]、炎症指标[C反应蛋白(CRP)、白介素-6(IL-6)、白介素-8(IL-8)]水平较治疗前均降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论加味大承气汤辅治SAP患者可提高疗效,促进患者康复,有利于改善胰腺微循环、减轻机体炎症反应。Objective To explore the effects of Jiawei Dachengqi Decoction(加味大承气汤)adjuvant therapy on patients with severe acute pancreatitis(SAP)and its influence on pancreatic microcirculation and inflammatory response.Methods A retrospective analysis was performed on clinical data of 112 SAP patients.A total of 48 patients were given routine treatment,and 64 patients were additionally given Jiawei Dachengqi Decoction on the basis of routine treatment.The efficacy,recovery,blood amylase content,pancreatic microcirculation indicators and inflammatory indicators were compared between the two groups.Results After seven days of treatment,the total effective rate of treatment in observation group was higher than that in control group(92.19%vs 79.17%)(P<0.05).After treatment,the first exhaust time,disappearance time of abdominal pain,recovery time of bowel sounds and ICU stay in the observation group were shorter than those in the control group(P<0.05).The levels of serum amylase,pancreatic microcirculation indicators[platelet activating factor(PAF),prothrombin time(PT),D-dimer]and inflammatory indicators[C-reactive protein(CRP),interleukin-6(IL-6),interleukin-8(IL-8)]in the two groups were decreased compared with those before treatment,and the levels in the observation group were lower than those in the control group(P<0.05).Conclusion Jiawei Dachengqi Decoction adjuvant therapy can improve the efficacy and promote the rehabilitation of SAP patients,and it is beneficial to improve pancreatic microcirculation and reduce the body′s inflammatory response.
