氨基酮戊酸光动力疗法治疗硬化萎缩性苔藓46例疗效观察

摘　　要：目的观察氨基酮戊酸光动力疗法治疗女阴硬化萎缩性苔藓的疗效和安全性。方法采用回顾性系列病例研究,受试者每2周进行1次氨基酮戊酸-光动力治疗,治疗2~4次,于治疗前、治疗后2、4、8周各随访1次,进行疗效和安全性评价。结果共有46例女阴硬化萎缩性苔藓的患者纳入疗效分析。治疗结束后,有效率为78.26%(36/46)。症状/体征总分在治疗后第2、4、8周分别为19.8±5.32、13.2±3.80和8.55±2.95,均明显低于治疗前的31.8±6.14,差异均有统计学意义(P<0.05)。其他疗效评估指标包括皮损面积、白斑、萎缩、糜烂、瘙痒评分在治疗后第8周均较治疗前改善明显,差异均有统计学意义(P均<0.05))。结论氨基酮戊酸光动力疗法治疗女阴硬化萎缩性苔藓疗效高且安全性好。Objective To evaluate the clinical efficacy and safety of aminolevulinic acid photodynamic therapy on lichen sclerosus et atrophicus.Methods This is a retrospective case series study.A total of 46 patients with lichen sclerosus et atrophicus were treated by aminolevulinic acid photodynamic therapy(PDT)once every two weeks for 2~4 sessions.They were examed before treatment and at 2,4,8 weeks after treatment to evaluate the efficacy and safety.Results The total effective rate of PDT treatment was 78.26%(36/46).The total scores of symptoms and signs on 2,4,and 8 weeks after treatment were 19.8±5.32,13.2±3.80,8.55±2.95,which were significantly lower than 31.8±6.14.There were statistically significant difference(P<0.05).Other efficacy evaluation indexes including skin lesion area,white spot,atrophy,erosion,and pruritus were all improved obviously at eight weeks after treatment.Conclusion PDT treatment is an effective and safe option for vulvar lichen sclerosus.
