摘  要:目的探讨MRI联合血清NSE水平评估新生儿高胆红素血症脑损伤的临床价值。方法选取我院2016年3月至2018年3月收治的新生儿高胆红素血症脑损伤患儿62例设为观察组,另选取60例于我院出生的健康新生儿作为对照组,进行血清NSE、TSB比较,对观察组患儿进行MRI检测。结果比较两组患儿血清TSB、NSE水平,观察组血清TSB、NSE水平均明显高于对照组(P<0.05);在观察组的62例患儿中进行MRI检测未见苍白球信号改变的27例(43.54%),有异常信号的有35例(56.45%)。将62例患儿根据MRI的检测结果分为MRI异常组和MRI正常组进行血清TSB、NSE水平比较,MRI异常组患儿血清NSE水平明显高于MRI正常组(P<0.05),MRI异常组患儿血清TSB水平明显高于MRI正常组(P>0.05)。结论血清NSE水平与MRI检测联合结果,可作为高胆红素脑损伤患儿的诊断指标,为治疗方案的制定和预后评判提供重要临床价值。Objective To explore the clinical value of MRI combined with serum NSE level in the assessment of brain injuryof neonatal hyperbilirubinemia.Methods 62 children with brain injury of neonatal hyperbilirubinemia admitted to our hospital from March 2016 to March 2018 were selected as the observation group,and 60 healthy newborns born in our hospital were selected as the control group.MRI was performed on the children in the observation group.Results Serum TSB and NSE levels were compared between the two groups.Serum TSB and NSE levels in the observation group were significantly higher than those in the control group(P<0.05).In the observation group,62 patients underwent MRI detection.There were 27 cases(43.54%)without changes of pallidal signal and 35 cases(56.45%)with abnormal signals.According to the MRI results,62 children were divided into MRI abnormal groups and MRI normal groups.The serum TSB and NSE levels were compared.The serum NSE level of the MRI abnormal group was significantly higher than that of the MRI normal group(P<0.05).Serum TSB levels in the MRI abnormal group were significantly higher than that in the MRI normal group(P>0.05).Conclusion The combination of serum NSE level and MRI test can be used as a diagnostic index for children with brain injury of hyperbilirubinemia,and provide significant clinical value for developing treatment plan and estimation of prognosis.
