摘 要:目的:通过植入前胚胎遗传学检测技术,初步研究胞浆内形态选择精子注射(IMSI)来源胚胎的遗传学安全性。方法:回顾分析2016年5月至2017年5月于大连市妇女儿童医疗中心接受植入前遗传学检测的83个周期患者,按受精方式不同分为IMSI组(n=48)和ICSI组(n=35),IMSI组、ICSI组成功活检囊胚144个和98个。分析两组患者的基本资料、临床数据、实验室数据及结局。结果:IMSI组和ICSI组患者的年龄、不孕年限、不孕类型、ART次数、基础内分泌、窦卵泡数、精液参数、MSOME法评估的A+B级精子细胞比例、获卵数、MII卵数、受精率、卵裂率、囊胚形成率比较,差异无统计学意义(P>0.05)。IMSI组的优胚率显著高于ICSI组(47.94%vs 33.02%,P<0.05);IMSI组的胚胎非整倍体率低于ICSI组,但差异无统计学意义(68.75%vs 71.42%,P>0.05)。两组的临床妊娠率、早期流产率、活产率、新生儿出生体重、性别比、Apgar评分、新生儿畸形率比较,差异均无统计学意义(P>0.05)。结论:IMSI可能改善卵裂期胚胎质量,且IMSI来源的胚胎非整倍体率较低,但并不能改善其助孕结局。Objective:To study the genetic safety of intracytoplasmic morphologically selected sperm injection(IMSI)derived embryos by preimplantation embryo genetic testing.Methods:The datas of 83 cycles undergone preimplantation genetic testing in the reproductive and genetic medical center of our hospital from May 2016 to May 2017 were analyzed retrospectively.According to the different fertilization methods,the cycles were divided into IMSI group(n=48)and ICSI group(n=35).144 blastocysts were successfully biopsied in IMSI group and 98 blastocysts in ICSI group.The basic data,clinical data,laboratory data and results of the two groups were compared.Results:There were no significant differences in age,duration of infertility,type of infertility,ART cycles,basal endocrine,antral follicles,Semen parameters,proportion of A+B sperm cells assessed by MSOME,retrieved oocytes,MII oocytes,fertilization rate,cleavage rate and blastocyst formation rate between IMSI group and ICSI group(P>0.05).The good quality embryo rate of IMSI group was significantly higher than that of ICSI group(47.94%vs 33.02%,P<0.05).The aneuploid rate of IMSI group was lower than that of ICSI group,but the difference was not statistically significant(68.75%vs 71.42%,P>0.05).There were no significant differences in clinical pregnancy rate,early abortion rate,live birth rate,birth weight,sex ratio,Apgar score and neonatal malformation rate between two groups(P>0.05).Conclusion:IMSI may improve the quality of cleavage embryos,and the rate aneuploid embryos derived from IMSI is lower,while it can not improve the outcome of assisted reproductive technology.
