摘 要:目的:观察恢刺“阳陵泉”穴对卒中后痉挛大鼠脑梗死体积和运动相关行为学的影响,为恢刺法在针灸临床治疗卒中后肢体痉挛提供依据。方法:45只SD大鼠随机分为假手术组(n=9)、模型组(n=18)和恢刺组(n=18)。采用改进的线栓法制作卒中后痉挛模型。术后第3天,恢刺组恢刺“阳陵泉”穴,留针30 min,1次/d,连续7 d。于造模前、后各时间点分别行Zea Longa神经功能损伤评分、改良Ashworth肌张力评分(MAS)和网屏实验运动功能评分。治疗后进行2,3,5-氯化三苯基四氮唑(TTC)脑切片染色观察脑梗死体积变化。结果:与假手术组比较,模型组大鼠造模后Zea Longa评分和MAS评分均显著升高(P<0.01),网屏实验评分显著降低(P<0.01),脑梗死体积比显著升高(P<0.01)。与模型组比较,恢刺组大鼠治疗后Zea Longa评分及MAS评分均显著降低(P<0.01),网屏实验评分显著升高(P<0.01),脑梗死体积比显著降低(P<0.01)。结论:恢刺法可缓解卒中后痉挛大鼠的肌痉挛,提高其运动功能,改善其脑缺血状态,发挥一定的神经功能保护作用。Objective:To observe the effects of lateral needling Yanglingquan(GB34)on cerebral infarction volumes and motor related behavior in rats with spasticity after stroke,so as to provide evidence of acupuncture therapy in treating post-stroke spasticity with lateral needling.Methods:A total of 45 healthy male SD rats were randomly divided into the sham-operated control group(n=9),the model group(n=18)and the lateral needling group(n=18).The stroke model was established by middle cerebral artery occlusion(MCAO).Three days after modeling,the lateral needling group was treated with lateral needling GB34(30 minutes,once daily for 7 days).Zea Longa scale,modified Ashworth scale(MAS)and the screen test were used to assess the neurological deficit,muscle tension and motor function respectively at several time points before and after the modeling.Cerebral infarction volumes were assessed by 2,3,5-triphenyl tetrazolium chloride(TTC)staining after the intervention.Results:Compared to those in the sham-operated control group,the Zea Longa scores and MAS scores were significantly increased(P<0.01),the screen test scores were significantly decreased(P<0.01),and the cerebral infarction volumes were significantly increased in the model group(P<0.01).After lateral needling intervention,the Zea Longa scores and MAS scores were significantly decreased(P<0.01),the screen test scores were remarkably increased(P<0.01),whereas the cerebral infarction volumes were significantly decreased(P<0.01).Conclusion:Lateral needling can relieve limb spasm and improve motor function in rats with post-stroke spasticity,as well as it can improve cerebral ischemia to exert certain neuroprotective effect.
