摘　　要：目的:探讨HCV-cAg在丙型肝炎中的诊断价值。方法:选取2017年2月-2019年2月于本院就诊的72例疑似丙型肝炎患者为研究对象。采用实时荧光定量PCR法检测HCV-RNA水平,分析HCV-RNA与HCV-cAg的检测结果,HCV-RNA不同载血量血清的HCV-cAg阳性率。结果:HCV-cAg检测的准确度88.9%(64/72),特异度92.7%(38/41),灵敏度83.9%(26/31)。HCV-RNA载血量血清数值<103 IU/mL、≥103 IU/mL且<104 IU/mL、≥104 IU/mL且<105 IU/mL、≥105 IU/mL且<106 IU/mL、>106 IU/mL患者的HCV-cAg阳性率分别为4.9%、75.0%、83.3%、91.7%、100%。结论:HCV-cAg诊断丙型肝炎的准确度、特异度及灵敏度均较高,HCV-RNA载血量血清数值越大,HCV-cAg阳性率越高,因此HCV-cAg的检测可作为HCV-RNA的补充检测,临床应用价值较高。Objective:To explore the diagnostic value of HCV-cAg in hepatitis C.Method:A total of 72 suspected hepatitis C patients admitted to our hospital from February 2017 to February 2019 were selected as the research subjects.The level of HCV-RNA was detected by real-time fluorescence quantitative PCR,and the detection results of HCV-RNA and HCV-CAG were analyzed,the positive rate of HCV-CAG in serum with different blood loads of HCV-RNA was analyzed.Result:The accuracy,specificity,and sensitivity of HCV-cAg detection were 88.9% (64/72),92.7% (38/41),83.9% (26/31).The positive rates of HCV-cAg were 4.9%,75.0%,83.3%,91.7%,100% in patients with HCV-RNA blood load and serum value <103 IU/mL,≥103 IU/mL and <104 IU/mL,≥104 IU/mL and <105 IU/mL,≥105 IU/mL and <106 IU/mL,>106 IU/mL,respectively,the higher the serum value of HCV-RNA blood load.Conclusion:The accuracy,specificity and sensitivity of HCV-cAg in the diagnosis of hepatitis C are high,the higher the serum value of HCV-cAg blood carrying volume,the higher the positive rate of HCV-cAg,therefore,the detection of HCV-cAg can be used as a supplementary detection of HCV-RNA,its clinical application value is high.
