体外冲击波使用不同频率冲击治疗足底筋膜炎的步态分析

摘　　要：目的:探讨不同频率体外冲击波治疗足底筋膜炎的步态变化。方法:符合足底筋膜炎诊断标准的患者160例。所有患者随机分为四组,每组40例。分别给予不同频率体外冲击波治疗,A组1 Hz,B组5 Hz,C组10 Hz,D组15 Hz。治疗前及治疗后1周进行足底压力和步态参数的变化情况,对数据进行统计学处理。结果:治疗前四组患者的动态足底压力差、跨步长、支撑相和摆动相比较,差异均无统计学意义(P>0.05);各组患者治疗前后的动态足底压力、跨步长、支撑相和摆动相比较,差异均有统计学意义(P<0.05);治疗后B组患者与其余三组患者比较,其动态足底压力差、跨步长、支撑相和摆动相比较,差异均有统计学意义(P<0.05),C组患者结果优于A组和D组(P<0.05),A组和D组患者结果比较差异均无统计学意义(P>0.05)。结论:体外冲击波治疗足底筋膜炎有效,其中5 Hz频率体外冲击波治疗足底筋膜炎效果较佳。Objective:To investigate the gait changes of different frequency extracorporeal shock wave in the treatment of plantar fasciitis.Method:There were 160 patients with plantar fasciitis,all patients were randomly divided into four groups,40 cases in each group.Different frequency extracorporeal shock wave therapy was given,group A of 1 Hz,group B of 5 Hz,group C of 10 Hz,group D of 15 Hz.The changes of plantar pressure and gait parameters before and 1 week after treatment were analyzed statistically.Result:There was no significant difference in dynamic plantar pressure difference,stride length,support phase and swing phase between the first four groups(P>0.05);there were significant differences in dynamic plantar pressure,stride length,support phase and swing phase before and after treatment in each group(P<0.05);after treatment,there were significant differences in dynamic plantar pressure difference,stride length,support phase and swing phase in group B compared with the other three groups difference(P<0.05);the results of group C were better than those of group A and group D(P<0.05);there was no significant difference between group A and group D(P>0.05).Conclusion:Extracorporeal shock wave is effective in the treatment of plantar fasciitis.
