免扩口法在后腹腔镜下重度积水无功能肾切除术中的临床应用分析

摘　　要：目的:探讨免扩口取标本法在后腹腔镜下重度积水无功能肾切除术中的临床应用价值。方法:回顾性分析2015年2月-2018年12月在潍坊市人民医院采用后腹腔镜下重度积水无功能肾切除术治疗的52例无功能肾患者的资料,根据标本取出方式的不同分为免扩口取标本组(n=22)和传统取标本组(n=30)。比较两组手术时间、切口长度、标本取出时间、术中出血量、切口并发症、术后住院时间、术后皮肤感觉异常程度、切口美容满意度的差异。结果:52例手术无中转开放手术,均获得成功,未出现死亡和重大并发症。两组的手术时间、标本取出时间、术中出血量、切口并发症发生率比较,差异均无统计学意义(P>0.05)。免扩口取标本组切口长度、住院时间均短于传统取标本组(P<0.05),术后皮肤感觉异常程度、切口美容满意度均优于传统取标本组(P<0.01)。结论:免扩口取标本法具有切口小、术后恢复快、术后皮肤感觉异常轻、切口美容满意度高等优点,值得临床推广应用。Objective:To evalua application of no-extending incision in retroperitoneal laparoscopic nephrectomy in the treatment of nonfunctioning kidney with severe hydronephrosis.Method:Retrospective analysis was performed on the data of 52 patients with nonfunctional kidney treated by retroperitoneal laparoscopic severe hydronephrotomy from February 2015 to December 2018 in Weifang People’s Hospital,according to the different methods of specimen extraction,the specimens were divided into non-extending specimen collection group(n=22)and traditional specimen collection group(n=30).Operative time,incision length,specimen retrieval time,estimated blood loss,incision complications,hospital stay,incision complications,postoperative skin sensory and cosmetic satisfaction were compared between the two groups.Result:All the 52 cases were successfully operated without death or major complications.There were no statistically significant differences between the two groups in operation time,sample removal time,intraoperative blood loss,and incidence of incision complications(P>0.05).The incision length and hospital stay in the non-extending specimen collection group were shorter than those in the traditional sample group(P<0.05),postoperative skin paresthesia degree and incision cosmetic satisfaction were all better than those of the traditional sample group(P<0.01).Conclusion:The method of non-extending specimen collection has the advantages of small incision,quick postoperative recovery,light postoperative skin sensation,and high satisfaction of incision beauty,which is worthy of clinical promotion and application.
