三维超声心动图在冠心病诊断中的应用

摘　　要：目的:探讨三维超声心动图在冠心病诊断中的应用价值。方法:选择2018年4月—2019年6月漯河市郾城区人民医院接收的疑似冠心病患者120例,所有患者采用三维超声心动图与经皮冠状动脉造影检查,以经皮冠状动脉造影检查结果为"金标准",分析三维超声心动图诊断冠心病的效能,同时分析三维超声心动图诊断结果与经皮冠状动脉造影诊断结果的一致性。结果:本组120例疑似为冠心病患者,经皮冠状动脉造影诊断出96例为阳性,占比80.00%;24例为阴性,占比20.00%。以经皮冠状动脉造影诊断结果为"金标准",三维超声心动图诊断冠心病的灵敏度为93.75%、特异度为83.33%、准确度为91.67%、阳性预测值为95.74%、阴性预测值为76.92%,三维超声心动图诊断结果与经皮冠状动脉造影诊断结果具有理想的一致性(Kappa=0.747)。结论:三维超声心动图在冠心病诊断中准确度高,可为冠心病的诊断、预防及治疗提供重要参照标准。Objective:To explore the application value of three-dimensional echocardiography in the diagnosis of coronary heart disease.Methods:120 patients with suspected coronary heart disease received in the hospital from April 2018 to June 2019 were selected.All patients were examined by three-dimensional echocardiography and percutaneous coronary angiography.The results of percutaneous coronary angiography were taken as the"gold standard"to analyze the efficacy of three-dimensional echocardiography in the diagnosis of coronary heart disease,and to analyze the diagnostic results of three-dimensional echocardiography and percutaneous coronary angiography Consistency.Results:Among 120 patients suspected to be coronary heart disease,96 cases were diagnosed as positive by percutaneous coronary angiography,accounting for 80.00%;24 cases were negative,accounting for 20.00%.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of three-dimensional echocardiography were 93.75%,83.33%,91.67%,95.74%,76.92%respectively.The diagnosis results of three-dimensional echocar diography and the diagnosis results of percutaneous coronary angiography had ideal consistency(Kappa=0.747).Conclusion:Three dimensional echocardiography has high accuracy in the diagnosis of coronary heart disease,which can provide an important reference standard for the diagnosis,prevention and treatment of coronary heart disease.
