静脉腔内激光治疗联合大隐静脉高位结扎术对下肢静脉曲张患者CIVIQ评分的影响

摘　　要：目的:探讨静脉腔内激光治疗(EVLT)联合大隐静脉高位结扎术对下肢静脉曲张患者慢性静脉功能不全问卷(CIVIQ)评分的影响。方法:回顾性收集郑州市第九人民医院2018年6月—2019年7月收治的77例下肢静脉曲张患者临床资料,根据其治疗方式分为对照组(n=41)和观察组(n=36)。对照组行大隐静脉高位结扎剥脱术治疗,观察组行EVLT联合大隐静脉高位结扎术治疗,比较两组围术指标、并发症发生率、CIVIQ评分及复发率。结果:观察组术中出血量、手术总用时、总住院时间均较对照组低,差异有统计学意义(P<0.05);观察组隐神经损伤、皮下血肿、患肢疼痛、切口愈合不良发生率均较对照组低,差异有统计学意义(P<0.05);两组术后6个月CIVIQ评分均较术前提高,且观察组较对照组高,差异有统计学意义(P<0.05);观察组术后6个月内复发率较对照组低,差异有统计学意义(P<0.05)。结论:下肢静脉曲张患者应用EVLT联合大隐静脉高位结扎术治疗可减少术中出血量,降低术后并发症及复发率,提高CIVIQ评分,促进患者康复。Objective:To investigate the impact of EVLT combined with high ligation of the great saphenous vein on CIVIQ score of patients with varicose veins of lower extremities.Methods:Clinical data of 77 patients with varicose veins of lower extremities admitted to the hospital from June 2018 to July 2019 were collected retrospectively.According to the treatment methods,they were divided into control group(n=41)and observation group(n=36).The control group was treated with high ligation of the great saphenous vein,and the observation group was treated with EVLT combined with high ligation of the great saphenous vein.Perioperative indicators,complication rate,CIVIQ score and recurrence rate were compared between the two groups.Results:Intraoperative blood loss,total operative time and total hospital stay in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of saphenous nerve injury,subcutaneous hematoma,pain in the affected limb and poor incision healing in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).Six months after operation,CIVIQ score of both groups improved compared with that before operation,and the score of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The recurrence rate 6 months after operation in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:EVLT combined with high ligation of the great saphenous vein can reduce intraoperative blood loss,reduce postoperative complications and recurrence rate,improve CIVIQ score,and promote patient recovery.
