摘  要:目的建立正常胎儿全心球形指数(GSI)Z评分正常范围。方法对849例孕11~40^(+6)周的正常胎儿进行产前超声检查,在标准切面测量GSI作为因变量,以胎体大小参数作为自变量,对GSI进行统计分析,计算Z评分正常范围。再应用已建立的正常范围值对一组可能引起GSI异常的病例进行评估。结果 (1)正常胎儿GSI的均数(M)和标准差(SD)均较为恒定。(2)GSI相对胎体大小参数的Z评分参数为:M=1.288,SD=0.097,Z评分=(GSI实测值-1.288)/0.097。(3)患病组胎儿GSI的Z评分均有不同程度降低,尤其以重型α-地贫最为显著。结论正常胎儿GSI的均数M及标准差SD在整个孕周均恒定,其测量及计算方法简便易行。当胎儿出现重型α-地贫等疾病时GIS值或其Z评分明显减低,作为一项新型超声指标评估此类疾病时可能具有较高的临床应用价值。Objective To establish reference ranges for the fetal Global Sphericity Index Z-score of normal fetus at every gestational age. Methods 849 normal fetuses with 11-40^(+6) weeks gestation were examined by prenatal ultrasound. The Global Sphericity Index was measured in the standard section as the dependent variable, and the fetal somatic sizes as the independent variable. The Global Sphericity Index of each group was statistically analyzed to obtain the normal range. The established normal range values were used to evaluate a group of patients who may cause the increase or decrease of Global Sphericity Index. Results(1)The mean(M) and standard deviation(SD) of GSI in normal fetuses were relatively constant.(2)The Z-score parameters of GSI relative to the size of the fetus: M=1.288, SD=0.097,Z-score=(GSI measured value-1.288)/0.097.(3)The Z-score of GSI in the fetuses were significantly reduced, especially in the homozygous α thalassemia-1. Conclusions The mean value and standard deviation of normal fetus GSI are constant throughout the gestational week, and the GSI value or its Z-score is significantly reduced when the fetus has intrauterine growth restriction or homozygous α thalassemia-1, which may have higher clinical application value as a new indicator to evaluate such diseases.
