乳腺颗粒细胞瘤的超声表现与病理特征对照分析

摘　　要：目的探讨乳腺颗粒细胞瘤的超声声像图表现及病理学特征。方法回顾性分析9例乳腺颗粒细胞瘤的超声资料及病理学特征,总结其声像图特征。结果 9例均为单发,肿瘤直径1.1~2.1 cm,均位于腺体边缘,超声表现为无包膜、形态不规则、边缘不规整的低回声团,团块内无微钙化和液化,6例肿块后方回声衰减,CDFI显示3例团块内及边缘少许血流信号。病理学特征为卵圆形、多角形细胞成片或成巢状排列,其间可见纤维间隔,胞质具有丰富的嗜酸性颗粒,细胞核小,无核分裂像,肿瘤细胞间可见灶性淋巴细胞浸润;肿瘤细胞向周围组织浸润性生长。结论乳腺颗粒细胞瘤多为良性肿瘤,其超声表现与乳腺癌相似,但其周边多无高回声晕,肿瘤内部无液化坏死及微钙化,CDFI团块多为乏血供,有助于鉴别诊断,最终确诊仍需依据病理免疫组织化学检测。Objective To investigate the ultrasonic manifestations and pathological features of granulosa cell tumor of the breast. Methods The ultrasonic data and pathological features of 9 cases of granulosa cell tumor of the breast were analyzed retrospectively. Results All the 9 patients had a single tumor with a diameter of 1.1-2.1 cm, all of which were located at the edge of the gland. The ultrasonic manifestations were hypoechoic masses with no capsule, irregular shape and irregular edges. No microcalcification and liquefaction were found in all masses.There was echo attenuation in the back of the mass in 6 cases, little blood flow signal in 3 cases. Under pathological examination, ovoid and polygonal cells were arranged in sheets or nests, with fibrous septa, abundant eosinophilic granules in cytoplasm, small nuclei, and no nucleation.Tumor cells infiltrate into surrounding tissue. Conclusions Granulosa cell tumor of the breast is mostly benign tumor, its clinical and ultrasonic manifestations are similar to that of breast cancer, but there is no edema zone around the tumor, no liquefaction necrosis and microcalcification inside the tumor, and CDFI shows masses are mostly lack of blood supply, which is helpful for differential diagnosis.
