基于美学理念的足底内侧岛状皮瓣修复足跟部软组织缺损

摘　　要：目的:探究基于美学理念的足底内侧岛状皮瓣修复足跟部软组织缺损的临床疗效。方法:选取2013年8月-2016年8月笔者医院进行足跟部软组织缺损修复的患者50例,患者缺损面积2.7cm×3.6cm~5.3cm×6.5cm,皮瓣面积3.5cm×4.6cm~7.5cm×9.4cm。随访36个月,随访过程中,采用视觉模拟评分量表(Visual analogue scale,VAS)评估皮瓣修复后患者的满意度,利用两点辨别觉评估感觉的敏感性,观察患者皮瓣的成活与否,同时与对侧健康足比较修复足的外观、温痛感、足部功能、神经感觉、并发症等症状。结果:全部患者足底内侧岛状皮瓣全部成活,与健侧足比较,患者在3~9个月后皮瓣色泽正常,质地与健侧足相似,外形美观,温感、痛感正常,患者负重行走稳定,足部功能恢复正常。患者行走过程中脚底无磨损、溃疡,足底部与足趾无发麻现象,且供区无并发症,神经感觉恢复较好。患者术后18个月,两点辨别觉30~40mm,平均(32.83±1.04)mm,外观评分为(9.03±0.78)分,患者满意度较高。结论:基于美学理念的足底内侧岛状皮瓣修复足跟部软组织缺损既能保证足跟部性能恢复的稳定性,又能保证外观的满意度,值得在临床推广使用。Objective To explore the clinical effect of repairing soft tissue defect of heel with medial plantar island flap based on aesthetic concept.Methods From August 2013 to August 2016,50 patients with soft tissue defect of heel were selected.The area of defect was 2.7cm×3.6cm-5.3cm×6.5cm,and the size of flap was 3.5cm×4.6cm-7.5cm×9.4cm.Patients were followed up for 36 months.during the follow-up,visual analogue scale(VAS)was used to evaluate the satisfaction of patients after flap repair,and two points of discrimination were used to evaluate the sensitivity of sensation.the survival of the skin flap was evaluated.at the same time,the appearance,warm pain,foot function,sensation,complications and other symptoms of the repaired foot were compared with the contralateral healthy foot.Results Compared with the healthy side foot,the skin flap was normal in color,similar in texture to the healthy side foot after 3 to 9 months,beautiful in appearance,temperature and pain were normal,stable in weight bearing and normal in foot function.In the course of walking,the patients had no wear,ulcers,no numbness in the foot and toe,no complications in the donor area,and good nerve recovery.patients 18 months after operation,two-point sense of discrimination 30-40mm,average(32.83±1.04)mm.The appearance score was(9.03±0.78),patient’s satisfaction was high.Conclusion The medial plantar island flap based on aesthetic concept repairing the soft tissue defect of the heel cannot only guarantee the stability of the heel,but also guarantee the satisfaction of the appearance.
