下法治疗黄疸探析

摘　　要：张仲景在《伤寒杂病论》中详述了黄疸的理、法、方、药,并提出“但利其小便”“当下之”等治疗原则。现代医学从胆红素的产生、代谢、排泄机制论述了黄疸的形成,中医下法能有效阻断肠肝循环、减压、促进胆汁排泄及肝细胞再生,是中医药治疗黄疸的关键。中医下法治疗黄疸能迅速改善症状,且无明显不良反应,但运用下法时,不能仅限于有“下之之证”才用,也勿受限于“痞、满、燥、实”等大实之证才用,需“观其脉证,知犯何逆,随证治之”。同时注意中病即止,以免使用过度伤及脾胃阳气。In Treatise on Cold Damage and Miscellaneous Diseases,Zhang Zhong-jing elaborated the principle-method-recipe-medicinal of jaundice,and put forward the treatment principles of"promoting urination"and"purgation".Modern medicine discusses the formation of jaundice from the production,metabolism and excretion mechanism of bilirubin.TCM purgative method can effectively block the intestinal and liver circulation,reduce pressure,promote bile excretion and liver cell regeneration,which is the key to the treatment of jaundice with TCM.TCM purgative method can improve the symptoms rapidly without obvious adverse reactions.However,the application of it should not be limited to the"symptoms in accordance with purgative method",nor limited to"swelling,fullness,dryness,excess"and other excessive syndromes.It is necessary to"observe the pulse syndrome,know what is the exact cause,and treat it according to the syndrome".At the same time,attention should be paid to stop using purgative method if it is effective in treating disease in order to avoid excessive injury to spleen-stomach yang qi.
