从胰岛素抵抗角度探讨脾与肠道菌群紊乱的相关性

摘　　要：从脾的功能角度研究胰岛素抵抗(insulin resistance,IR),其核心病机为脾不散精,胃热亢盛;从肠道菌群角度研究IR,其病理机制为机体长期摄入高热量食物,导致肠道内厚壁菌门/拟杆菌门值升高、乙酸盐含量增加、益生菌比例降低和丁酸盐含量减少以及肠道屏障受损。中医采用清除胃热和补益健脾的治法可改善肠道菌群结构,有效提高胰岛素敏感性,说明了清除胃热和补益健脾的治法对治疗肠道菌群紊乱所致的IR具有一定疗效。但是对于肠道菌群、菌门之间比例是否改变、菌群是否发生移位、移行到哪些部位从而引发IR的研究鲜有报道,需要更多的实验研究来发现肠道菌群紊乱所致IR的机制以及脾与IR的关系。The core pathogenesis of insulin resistance(IR)is that spleen does not disperse essence and stomach heat is hyperactive from the perspective of spleen.While from the perspective of intestinal flora,it is found that long-term high calorie intake will lead to the increase of the value of firmicum/bacteroides,the increase of acetate content,the decrease of probiotics and butyrate content,as well as the damage of intestinal barrier,resulting in IR.In TCM,the treatment of clearing stomach heat and tonifying spleen can improve the structure of intestinal flora and effectively increase insulin sensitivity.It shows that these methods have certain reference value for the treatment of IR caused by intestinal flora disorder.However,there are few reports on the changes of the proportion of intestinal microflora,whether the flora has shifted or moved to certain parts and lead to IR.Therefore,in the future,more experimental studies are needed to find out the mechanism of IR caused by disturbance of intestinal flora and the correlation between spleen and IR.
