针刀治疗难治性面瘫临床研究进展

摘　　要：针刀治疗难治性面瘫方法有:单纯针刀治疗、针刀结合电针、针刀结合药物、针刀结合推拿、针刀结合拔罐等,均能有效改善难治性面瘫的症状。针刀治疗难治性面瘫作用机制为:松解粘连,解除压迫;改善循环,消除水肿。在今后的研究中,针刀治疗难治性面瘫可在难治性面瘫的诊断和疗效评价中引入肌电图等客观数据,并针对患者具体临床表现开展高质量的随机对照试验研究;针刀作为一种有创治疗,容易损伤施术部位的神经、血管、肌肉等组织,造成不良后果。为了减少此类不良事件的发生,今后应加强针刀治疗难治性面瘫的最佳介入时间、操作次数及频率的研究,还可借助现代医疗仪器对针刀在头面部的入路进行精准定位。The treatment of refractory facial paralysis methods with acupotomy include simple acupotomy,acupotomy combined with electroacupuncture,acupotomy combined with medicine,acupotomy combined with massage,acupotomy combined with cupping,etc.which all can effectively improve the symptoms of refractory facial paralysis.The mechanism of acupotomy in the treatment of refractory facial paralysis is as follows:to release adhesion and relieve compression,improve blood circulation and eliminate edema.In the future research,objective data such as electromyography in the diagnosis and efficacy evaluation can be introduced,and high-quality randomized controlled trials can be carried out according to the specific clinical manifestations of patients.As an invasive treatment,acupotomy is easy to damage the nerves,blood vessels,muscles and other tissues of the operation site,resulting in adverse consequences.In order to reduce the occurrence of such adverse events,we should strengthen the research on the best intervention time,operation times and frequency of acupotomy in the treatment of refractory facial paralysis in the future.We can also use modern medical instruments to accurately locate the craniofacial approach when using acupotomy.
