摘　　要：目的:探讨美罗培南注射剂联合头孢哌酮钠舒巴坦钠注射剂治疗内科重症监护室(MICU)脓毒症休克并肾功能不全的效果。方法:随机抽取2018年3月-2019年3月本院收治的MICU脓毒症休克并肾功能不全患者68例。按照随机数字表法分为观察组和对照组,每组34例。观察组实施美罗培南注射剂联合头孢哌酮钠舒巴坦钠注射剂治疗,对照组实施头孢哌酮钠舒巴坦钠注射剂治疗,比较两组治疗效果。结果:治疗前,两组血清指标以及APACHE-Ⅱ评分比较,差异均无统计学意义(P>0.05);治疗后,观察组血清指标、APACHE-Ⅱ评分均低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组尿量、LAC水平比较,差异均无统计学意义(P>0.05);治疗后,观察组尿量大于对照组,LAC低于对照组,差异均有统计学意义(P<0.05)。观察组的总有效率显著高于对照组,差异有统计学意义(P<0.05)。观察组血管活性药物总量均显著低于对照组,差异均有统计学意义(P<0.05)。结论:美罗培南注射剂联合头孢哌酮钠舒巴坦钠注射剂治疗MICU脓毒症休克并肾功能不全的效果显著,可降低机体炎性反应,缓解症状,提升治疗效果。Objective:To investigate the effect of Meropenem Injection combined with Cefoperazone Sodium and Sulbactam Sodium Injection in the treatment of septic shock combined with renal insufficiency in MICU.Method:A total of 68 patients with MICU septic shock and renal insufficiency admitted to our hospital from March 2018 to March 2019 were randomly selected.According to random number table method,they were divided into observation group and control group,34 cases in each group.The observation group was treated with Meropenem combined with Cefoperazone Sodium and Sulbactam Sodium Injection,while the control group was treated with Cefoperazone Sodium and Sulbactam Sodium Injection,and the therapeutic effects of the two groups were compared.Result:Before treatment,there were no significant differences in serum indexes and APACHE-Ⅱscores between the two groups(P>0.05).After treatment,the serum indexes and APACHE-Ⅱscores of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in urine volume and LAC level between the two groups(P>0.05).After treatment,the urine volume of the observation group was higher than that of the control group,LAC was lower than that of the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The total amount of vasoactive drugs in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Meropenem Injection combined with Cefoperazone Sodium and Sulbactam Sodium Injection has a significant effect in the treatment of MICU septic shock combined with renal dysfunction,which can reduce the inflammatory response of the body,relieve the symptoms and improve the therapeutic effect.
