游离股前外穿支皮瓣修复糖尿病足部溃疡创面的应用效果

摘　　要：目的分析对糖尿病存在足部溃疡的患者实施游离股前外穿支皮瓣修复术干预的治疗效果及价值。方法实验选取2017年1月—2020年4月期间入院接受治疗的糖尿病并发足部溃疡患者48例为研究样本,对其创面均应用游离股前外穿支皮瓣修复治疗,在术前、术中、术后等不同阶段后实施对症干预手段,进行知识宣教、心理干预、手术完善检查、清创干预、血糖控制、抗感染及抗凝活血等诸多方面的术前准备,术中根据患者实际情况进行皮瓣的选择、择取与覆盖,术后观察患者的皮瓣血运情况及血糖水平。对皮瓣的存活情况及溃疡复发情况进行统计分析。结果所有患者的皮瓣存活率为100%,且经半年的随访,患者的皮瓣弹性质地较好,仅有1例发生溃疡的轻度复发,在处理后完全好转。结论对糖尿病足部溃疡创面实施游离股前外穿支皮瓣修复术治疗,价值明显,值得在临床中推广实施。Objective To analyze the therapeutic effect and value of free anterior thigh perforator flap repair for diabetic patients with foot ulcers.Methods In the experiment,48 cases of diabetic foot ulcer patients who were admitted to the hospital for treatment from January 2017 to April 2020 were selected as the study samples.All their wounds were repaired with free anterior thigh external perforator flaps.Before surgery,symptomatic interventions are implemented at different stages such as intraoperative and postoperative,and preoperative preparations are carried out in many aspects such as knowledge education,psychological intervention,surgical complete inspection,debridement intervention,blood glucose control,anti-infection,anticoagulation and blood circulation.The actual situation is to select,select and cover the skin flap,and observe the patient’s skin flap blood supply and blood glucose level after the operation.The survival of the flaps and the recurrence of ulcers were statistically analyzed.Results The survival rate of the flaps of all patients was 100%,and after half a year of follow-up,the flaps of the patients had good elastic texture.Only one case had a mild recurrence of ulcer,which was completely improved after treatment.Conclusion The repair of free anterior and external perforator skin flaps for diabetic foot ulcers has obvious value and is worthy of clinical application.
