比较阴式手术与宫腹腔镜联合手术对剖宫产术后子宫瘢痕憩室的治疗效果

摘　　要：目的对比阴式与宫腹腔镜子宫瘢痕憩室修补术在剖宫产术后子宫瘢痕憩室治疗中的临床治疗效果。方法方便选取该院2015年12月—2018年12月收治的剖宫产术后子宫瘢痕憩室患者70例,将患者随机分为对照组(n=35)、观察组(n=35)。对照组采用宫腹腔镜手术治疗,观察组行阴式手术治疗。观察对比两组患者的手术有效率、手术情况、影像学变化及术后1年经期恢复情况。结果观察组与对照组手术有效率对比差异无统计学意义(P>0.05);观察组患者手术时长(69.66±15.47)min、肛门排气时间(23.67±0.64)h、手术费用(5585.36±758.66)元显著低于对照组,差异有统计学意义(t=11.943、17.323、13.990,P<0.001);治疗后两组患者超声图像改善例数比较差异无统计学意义(P>0.05);两组术后异常阴道流血、经期时长>1周例数比较差异无统计学意义(P>0.05)。结论阴式手术治疗剖宫产术后子宫瘢痕憩室疗效确切,术后恢复速度快,手术式较为安全简单,且费用较低,临床上值得进行更多推广应用。Objective To compare the clinical effects of vaginal and hystero-laparoscopic uterine scar diverticulum repair in the treatment of uterine scar diverticulum after cesarean section.Methods Seventy patients with uterine scar diverticulum after cesarean section admitted to the hospital from December 2015 to December 2018 were conveniently selected.The patients were randomly divided into control group(n=35)and observation group(n=35).The control group was treated with hystero-laparoscopic surgery,and the observation group was treated with vaginal surgery.Observation and comparison of the surgical efficiency,surgical conditions,imaging changes and 1-year menstrual recovery of the two groups of patients.Results There was no statistically significant difference between the observation group and the control group in the operation efficiency(P>0.05);the observation group had the operation time(69.66±15.47)min,the anal exhaust time(23.67±0.64)h,and the operation cost(5585.36±758.66)yuan was significantly lower than the control group,and the difference was statistically significant(t=11.943,17.323,13.990,P<0.001);after treatment,there was no statistically significant difference in the number of improved ultrasound images between the two groups(P>0.05);There was no statistically significant difference in the number of cases of abnormal vaginal bleeding and menstrual duration>1 week between the two groups after operation(P>0.05).Conclusion Vaginal surgery is effective in treating uterine scar diverticulum after cesarean section,the recovery speed is fast,the operation is safer and simpler,and the cost is low.It is worthy of more clinical application.
