全膝关节置换术治疗膝关节内外翻畸形的效果研究

摘　　要：目的分析全膝关节置换术治疗膝关节内外翻畸形的效果。方法于2018年6月—2019年12月便利抽取在该院进行治疗的128例膝关节内外翻畸形患者,选用随机分配法,对患者进行分组,共分为两组,组别名称为观察组(n=64)及对照组(n=64),对观察组患者行全膝关节置换术治疗,对对照组患者实施手法治疗,对比两组患者膝关节指标(膝关节功能评分、活动范围评分、ROM评分),膝关节疼痛程度(轻度疼痛、中度疼痛、重度疼痛)。结果两组膝关节指标比较,观察组患者治疗后膝关节指标(膝关节功能评分、活动范围评分、ROM评分)分别为(12.24±2.47)分、(13.45±2.71)分、(104.91±15.45)分,高于对照组膝关节指标(3.72±2.26)分、(8.45±3.36)分、(78.41±16.35)分,差异有统计学意义(t=20.359、9.266、9.424,P<0.001),患者关节疼痛程度对比,观察组患者治疗后轻度疼痛3例(4.69%)、中度疼痛0例(0.00%)、重度疼痛0例(0.00%),相比对照组患者轻度疼痛20例(31.25%)、中度疼痛24例(37.50%)、重度疼痛20例(31.25%),差异有统计学意义(χ^(2)=15.318、28.900、23.183,P<0.001)。结论膝关节内外翻畸形患者应用全膝关节置换术治疗,能够降低患者的肢体疼痛,提升患者的活动能力,治疗效果显著,且患者治疗后生活质量明显改善,值得在治疗膝关节内外翻畸形中应用。Objective To analyze the effect of total knee arthroplasty in the treatment of knee varus deformity.Methods From June 2018 to December 2019,128 patients with varus and valgus deformity of knee joint treated in the hospital were conveniently selected The patients were divided into two groups using the random allocation method.The group alias was called observation group(n=64)and control group(n=64).Patients in the observation group were treated with total knee arthroplasty,and patients in the control group were treated with manual treatment.The knee joint indexes(knee joint function score,range of motion score,ROM score),knee joint pain degree(mild pain,moderate pain,severe pain)between the two groups were compared.Results The knee joint indexes between the two groups were compared.The knee joint indexes(knee joint function score,range of motion score,ROM score)of the observation group after treatment were(12.24±2.47)points,(13.45±2.71)points,(104.91±15.45)points which were higher the control group’s knee joint index(3.72±2.26)points,(8.45±3.36)points,(78.41±16.35)points,the difference was statistically significant(t=20.359,9.266,9.424,P<0.001),the patients in the treatment of joint pain were compared,and the observation group had 3 cases of mild pain(4.69%),0 cases of moderate pain(0.00%),0 cases of severe pain(0.00%)after treatment compared with 20 cases of mild pain(31.25%),24 cases of moderate pain(37.50%),and 20 cases(31.25%)of severe pain in the control group,the difference was statistically significant(χ^(2)=15.318,28.900,23.183,P<0.001).Conclusion The application of total knee arthroplasty for patients with knee valgus deformity can reduce the patient’s limb pain,improve the patient’s mobility,the treatment effect is significant,and the patient’s quality of life after treatment is significantly improved,and it is worthwhile to apply in treating knee valgus deformity.
