摘  要:目的通过检测正常宫颈组织、宫颈低级别上皮内瘤变(LSIL)组织、宫颈高级别上皮内瘤变(HSIL)组织和宫颈鳞状细胞癌组织中自噬基因LC3Ⅱ和Beclin1的表达水平,分析LC3Ⅱ、Beclin1与宫颈鳞状细胞癌临床病理特征之间的关系及其相关性,探讨自噬基因在宫颈鳞状细胞癌发生、发展中的作用及其对患者生存的影响。方法选取2015年6月-2019年6月在甘肃省人民医院妇产科接受诊治且临床资料完整的宫颈标本190例,其中正常宫颈组织20例、LSIL组织30例、HSIL组织48例(CIN 22例、CINⅢ26例)、宫颈鳞状细胞癌组织92例。免疫组化染色法检测LC3Ⅱ、Beclin1的表达水平,并采用Kaplan-Meier生存曲线比较分析患者总生存期。结果 LC3Ⅱ、Beclin1阳性表达率随宫颈细胞病变级别的升高渐呈下降趋势,正常宫颈组织、LSIL和HSIL组织、宫颈鳞状细胞癌组织比较,差异有统计学意义,而正常宫颈组织和LSIL组织间差异无统计学意义。LC3Ⅱ、Beclin1在宫颈鳞状细胞癌中的表达无相关性。Beclin1、LC3Ⅱ不是影响宫颈鳞状细胞癌总生存时间的独立危险因素,但患者的生存率随着Beclin1、LC3Ⅱ阳性表达率的增高而呈增高趋势。结论 LC3Ⅱ、Beclin1在宫颈细胞癌变过程中表达逐渐下调,癌变完成后自噬能力显著降低,自噬活性的改变可能与宫颈鳞状细胞癌的发生、发展相关。Objective To analyze the relationship between autophagy gene(LC3Ⅱ and Beclin1)and some clinical pathological features of cervical squamous cell carcinoma and the correlation between the LC3Ⅱ and Beclin1 gene through testing and compare autophagy gene LC3Ⅱ and Beclin1 expression level in normal cervical tissue,low cervical intraepithelial neoplasia(LSIL)and high cervical intraepithelial neoplasia(HSIL)and cervical cancer tissue,then discuss the role of autophagy gene LC3Ⅱ and Beclin1 in cervical cancer occurrence and development.Methods A total of 190 patients with cervical tissue from the Department of Obstetrics and Gynecology of Gansu Provincial Hospital were selected,including 20 cases of normal cervical tissue,30 cases of cervical LSIL,48 cases of cervical HSIL(22 cases CIN and 26 cases CIN)and 92 cases of cervical squamous cancer tissue.The expression levels of LC3Ⅱ and Beclin1 were detected through adopting the immunohistochemical staining,comparing and analyzing the patients'overall survival by using the Kaplan-Meier survival curve.Results The positive expression rate of LC3Ⅱ and Beclin1 decreased gradually with the aggravation of cervical cell lesion level,while the negative expression rate increased gradually.There was significant difference between normal cervical tissue,LSIL,HSIL and cervical squamous cell carcinoma group,while no statistical difference among normal cervical tissue and LSIL.The expression of LC3Ⅱ and Beclin1 in cervical squamous cell carcinoma bore no correlation.Beclin1 and LC3Ⅱ were not independent risk factors affecting the total survival time of cervical squamous cell carcinoma,the survival rate of patients increased with the increase of Beclin1 and LC3Ⅱ positive rate.Conclusion LC3Ⅱ and Beclin1 were gradually down-regulated in the process of cervical cancer,and the autophagy ability was significantly reduced after the cancer was completed.The change of autophagy activity may be related to the occurrence and development of cervical squamous cell carcinoma.
