摘　　要：目的:研究营养不良型大疱表皮松解症(DEB)手畸形患者双手瘢痕挛缩松解术后手功能的影响因素。方法:回顾性分析41例行双手瘢痕挛缩松解术DEB患者,记录可能影响患者手功能的因素及患者术后手功能受限情况,根据术后2年手功能评分,将患者评分等级为优、良的设为手功能未受限组,等级评定为差、劣的设为手功能受限组,比较两组患者的基本情况、术后手部支具的使用情况及常见因素,判断患者术后手功能的影响因素。结果:41例患者中手功能受限组21例,未受限组20例。两组患者年龄、性别、民族、身体质量指数、生活能力评估记录指数、营养状态、血红蛋白浓度、血糖、凝血功能间差异均无统计学意义(P值均>0.05),术后手部支具生活能力评估记录的使用情况差异有统计学意义(χ^(2)=12.95,P<0.05)。结论:DEB患者外科手术治疗后手部支具的使用是影响患者术后手功能的重要因素。Objective:To investigate the influencing factors of postoperative hand function in patients with dystrophic epidermolysis bullosa.Methods:A retrospective analysis was performed in 41 patients with dystrophic epidermolysis bullosa with hands contractures deformities.According to the hand function scores 2 years after surgery,patients were classified into unrestricted hand function and restricted hand function groups.Basic conditions,use of postoperative hand braces and other common factors were compared between the two groups to determine the influencing factors of postoperative hand function.Results:According to hand function scores,subjects with unrestricted hand function and restricted hand function were 20 and 21 patients,respectively.There were no significant differences in age,gender,race,BMI,BARTHEL index,nutritional status,hemoglobin concentration,blood glucose and coagulation function between the two groups(P>0.05),while there were significant differences in the use of hand support between the two groups(χ^(2)=12.95,P<0.05).Conclusion:The application of hand brace after surgical treatment is an important factor that affects the postoperative hand function in patients with dystrophic epidermolysis bullosa hands complicated with contracture deformities.
