摘 要:目的监测重症颅脑损伤患者动态血清硫氧还蛋白1(TRX1)与颅内压(ICP)的变化情况,分析TRX1水平与ICP的相关性。方法回顾性分析2019-01—2020-02濮阳市人民医院确诊并完成治疗的40例重症颅脑损伤患者的临床资料,将其纳入观察组,选取同期40例轻中型颅脑损伤患者纳入对照组,监测并记录2组患者颅脑损伤后12 h、24 h、48 h血清TRX1水平与ICP水平,检验血清TRX1水平与ICP的相关性,分析重症颅脑损伤后不同时点血清TRX1对患者ICP的影响。结果入院后12 h、24 h,2组患者ICP、TRX1水平呈升高趋势发展,但在入院后48 h 2组患者ICP、TRX1水平均有回降,各时点观察组ICP、TRX1水平均高于对照组,差异有统计学意义(P<0.05);经双变量Pearson直线相关性检验显示,不同时点颅脑损伤患者的ICP水平与TRX1水平均呈正相关(r>0,P<0.05);一般线性回归分析显示,血清TRX1水平过表达是重症颅脑损伤患者ICP水平的影响因素(P<0.05)。结论重症颅脑损伤患者ICP显著升高,可能与患者血清TRX1的过表达有关,未来可以考虑通过动态监测血清TRX1水平判断其ICP升高情况或风险,以期为重症颅脑损伤早期治疗提供新思路。Objective To dynamically monitor the changes of serum thioredoxin 1(TRX1)and intracranial pressure(ICP)in patients with severe craniocerebral injury,and to analyze the correlation between TRX1 level and ICP of patients.Methods The clini⁃cal data of 40 patients with severe craniocerebral injury who diagnosed and completed the treatment in the Puyang People’s Hospital from January 2019 to February 2020 were included in observation group,and the clinical data of 40 patients with mild-moderate cra⁃niocerebral injury were included in control group.The serum TRX1 level and ICP level after craniocerebral injury for 12h,24h and 48h in two groups were monitored and recorded,and the correlation between serum TRX1 level and ICP was examined.The effect of serum TRX1 at different time after severe craniocerebral injury on ICP was analyzed.Results After admission for 12h and 24h,the levels of ICP and TRX1 in the two groups showed an increasing trend,but the levels of ICP and TRX1 in two groups declined after ad⁃mission for 48h,and the levels of ICP and TRX1 in observation group were higher than those in control group at different time(P<0.05).The bivariate Pearson linear correlation test showed that the ICP level was positively correlated with TRX1 level in patients with craniocerebral injury at different time(r>0,P<0.05).The results of general linear regression analysis show that the over-ex⁃pression of serum TRX1 level was the influencing factor of ICP level in patients with severe craniocerebral injury(P<0.05).Conclu⁃sion The significant increase of ICP in patients with severe craniocerebral injury may be related to the over-expression of serum TRX1 in patients.In the future,dynamic monitoring of serum TRX1 levels in patients with severe craniocerebral injury may be consid⁃ered to determine the increase or risk of ICP in patients,so as to provide new ideas for early treatment of severe craniocerebral injury.
