摘  要:目的探讨GeXP多重RT-聚合酶链反应(polymerase chain reaction,PCR)诊断小儿肺炎支原体(mycoplasma pneumoniae,MP)感染性脑炎的价值。方法采用回顾性研究,选取2017-05—2020-05郑州儿童医院收治的85例初诊为感染性脑炎患儿为研究对象,采集患儿脑脊液标本进行肺炎支原体检测,采用GeXP多重RT-PCR技术进行诊断。患儿均经脑电图、实验室检查确诊,分析GeXP多重RT-PCR的诊断效能。结果85例患儿中24.71%为细菌感染,3.53%为病毒感染,71.76%为支原体感染。61例患者进行MP检测,抗体颗粒凝集法检测显示15例阴性,46例阳性;GeXP多重RT-PCR法检测显示阳性55例,阴性6例,GeXP多重RT-PCR法检测效能与抗体颗粒凝集法比较,差异有统计学意义(P<0.05)。GeXP多重RT-PCR法检测中6例漏诊患者的脑脊液生化检测均为MP阳性株,血清肺IgM滴度1∶160~1∶640。结论GeXP多重RT-PCR可有效诊断MP感染性脑炎,且具有较高的灵敏度、准确率等优势,有助于临床医师及时确诊疾病。Objective To explore the value of GeXP-based multiplex RT-polymerase chain reaction(PCR) in the diagnosis of mycoplasma pneumoniae(MP) with infectious encephalitis in children.Methods Adopting a retrospective research,85 children patients who were newly diagnosed with infectious encephalitis in this hospital from May 2017 to May 2020 were selected as the research subjects.Cerebrospinal fluid samples were collected for detection of mycoplasma pneumonia,and GeXP-based multiplex RT-PCR technology was used for diagnosis.All children patients were diagnosed by electroencephalogram(EEG) and laboratory examination,and the diagnostic efficiency of GeXP-based multiplex RT-PCR was analyzed.Results There were 24.71% of bacterial infections,3.53% of viral infections,and 71.76% of mycoplasma infections.Sixty-one patients were tested for MP,and antibody particle agglutination method showed 15 negative cases and 46 positive cases,and GeXP-based multiplex RT-PCR method showed 55 positive cases and 6 negative cases.There was a statistically significant difference in the detection efficiency of GeXP-based multiplex RT-PCR method and antibody particle agglutination method(P<0.05).Among GeXP-based multiplex RT-PCR detection,6 cases with missed diagnosis showed MP-positive strains in cerebrospinal fluid biochemical analysis,and the serum lung IgM titer was 1:160-1:640.Conclusion GeXP-based multipl ex RT-PCR can effectively diagnose MP with infectious encephalitis,and has the advantages of high sensitivity and accuracy rate,and it can help clinicians to diagnose the disease in time.
