老年阿尔茨海默病患者发生挑战性行为的影响因素分析

摘　　要：目的分析老年阿尔茨海默病(AD)患者挑战性行为发生的影响因素,旨在为未来老年AD患者挑战性行为发生风险预测及干预提供指导。方法回顾性分析2017-01—2019-01在郑州市第九人民医院确诊并接受治疗的95例老年AD患者临床资料。评估患者的挑战性行为发生情况,统计患者一般资料,包括性别、年龄、婚姻状况、合并其他疾病、自理能力、受教育程度、病程,采用Logistic回归分析,经单因素与多因素分析老年AD患者挑战性行为发生的影响因素。结果67例发生挑战性行为,发生率为70.53%;初步比较发生挑战性行为与未发生挑战性行为患者的基线资料,将差异显著的变量纳入单因素分析,后建立多元回归模型行多因素分析,结果显示,年龄>75岁、合并≥3种其他疾病、初中及以下学历、自理能力完全依赖是老年AD患者挑战性行为发生的影响因素(OR>1,P<0.05)。结论年龄>75岁、合并≥3种其他疾病、初中及以下学历、自理能力完全依赖的老年AD患者更容易发生挑战性行为,临床应重视对患者的早期干预,以减少挑战性行为的发生。Objective To analyze the influencing factors of challenging behaviors in elderly patients with Alzheimer’s disease(AD),aiming to provide guidance for the prediction and intervention of the risk of challenging behaviors in elderly AD patients in the future.Methods The clinical data of 95 elderly patients with AD who were diagnosed and treated in the hospital from January 2017 to January 2019 were retrospectively analyzed.Assessed the occurrence of the patient’s challenging behaviors,counted the gen⁃eral information of patients,including gender,age,marital status,combined other diseases,self-care ability,education level,course of illness,Logistic regression analysis,single factor and multivariate analysis were used to test the influencing factors of chal⁃lenging behaviors in elderly AD patients.Results Among 95 elderly AD patients,67 had challenging behaviors,the incidence was 70.53%;after initially comparing the baseline data of patients with challenging behaviors and those without challenging behaviors,the variables with significant differences were included in the univariate analysis,and then a multiple regression model was estab⁃lished and multivariate analysis was performed.The results showed that age>75 years old,combined with≥3 other diseases,junior high school education and below,and complete dependence on self-care ability were the influencing factors of challenging behaviors in elderly AD patients(OR>1,P<0.05).Conclusion Elderly AD patients who are over 75 years old,with≥3 other diseases,have a junior high school education or below,and are completely dependent on self-care ability are more likely to have challenging behaviors.The clinic should pay attention to the early intervention of such elderly AD patients to reduce the occurrence of challeng⁃ing behaviors.
