老年不同时期痛风性关节炎超声半定量评分与临床表现的相关性分析

摘　　要：目的探讨老年不同时期痛风性关节炎超声半定量评分与临床表现的相关性。方法选取老年痛风性关节炎患者86例,其中急性发作期(A组)39例,非急性发作期(B组)47例,患者进行肌骨超声检查,记录患者的临床表现,包括疼痛指数与患者总体评估。采用线相关法,分析不同时期老年痛风性关节炎超声半定量评分与临床表现的相关性。结果 A组较B组病程短(P<0.05),双轨征及痛风石的数量较B组少(P<0.05)。A组的滑膜炎评分、疼痛指数与患者总体评估评分,均较B组高(P均<0.05)。A组患者超声半定量评分与疼痛指数及患者总体评估呈正相关(r=0.868,P<0.001;r=0.803,P<0.001)。结论利用肌骨超声半定量评分可精准、客观评估老年患者不同时期痛风性关节炎的疾病活动度,增强老年痛风患者对疾病的认识,使其及时接受有效治疗。Objective To investigate the correlation between ultrasound semi-quantitative scores and clinical manifestations of elderly patients with gouty arthritis in different periods.Methods 86 elderly patients with gouty arthritis were selected,including 39 cases in acute attack stage(group A) and 47 cases in non acute attack stage(group B).The patients were examined by musculoskeletal ultrasound,and their clinical manifestations were recorded,including Visual Analogue Scale(VAS) score and Patient Global Assessment(PGA) score.Linear correlation method was used to analyze the correlation between ultrasound semi-quantitative scores and clinical manifestations of elderly patients with gouty arthritis in different periods.Results The course of disease of group A was shorter than that of group B(P<0.05),and the number of double contour sign and tophi of group A were less than those of group B(P<0.05).The scores of synovitis,VAS and PGA score of group A were higher than those of group B(P<0.05).In group A,the ultrasound semi-quantitative score was positively correlated with pain index and PGA score(r=0.868,P<0.001;r=0.803,P<0.001).Conclusion The disease activity can be accurately and objectively evaluated by semiquantitative ultrasound scores in elderly patients,which can help the elderly patients with gout to understand the disease well.
