雷公藤多苷片联合清热止血方对紫癜性肾炎患者的临床疗效

摘　　要：目的观察雷公藤多苷片联合清热止血方对紫癜性肾炎患者的临床疗效。方法168例患者随机分为对照组(55例)和观察组(113例),对照组给予强的松联合贝那普利、双嘧达莫,观察组给予雷公藤多苷片联合清热止血方,疗程12周。检测临床疗效、T细胞亚群(CD3、CD4、CD8、CD4/CD8)、免疫球蛋白(IgA、IgG、IgM)、补体(C3、C4)、不良反应发生率变化。结果4周末,观察组减少蛋白尿疗效优于对照组(P<0.05);12周末,观察组减少尿红细胞疗效优于对照组(P<0.05)。治疗后,2组CD3、CD4、CD4/CD8升高(P<0.05),CD8、IgA降低(P<0.05),以观察组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论雷公藤多苷片联合清热止血方能安全有效地降低紫癜性肾炎患者蛋白尿水平,改善免疫状态。AIM To observe the clinical effects of Tripterysium Glycosides Tablets combined with Qingre Zhixue Decoction on patients with Henoch-Schönlein purpura nephritis.METHODS One hundred and sixty-eight patients were randomly assigned into control group(55 cases)for 12-week administration of prednisone combined with benazepril and dipyridamole,and observation group(113 cases)for 12-week administration of Tripterysium Glycosides Tablets combined with Qingre Zhixue Decoction.The changes in clinical efficacy,T cell subsets(CD3,CD4,CD8,CD4/CD8),immunoglobulins(IGA,IgG,IgM),complements(C3,C4)and incidence of adverse reactions were detected.RESULTS At the end of 4 weeks,the observation group demonstrated better efficacy in reducing proteinuria than the control group(P<0.05).At the end of 12 weeks,the observation group displayed better efficacy in reducing urine red blood cells than the control group(P<0.05).After the treatment,the two groups exhibited increased CD3,CD4 and CD4/CD8(P<0.05),and decreased CD8 and IgA(P<0.05),especially for the observation group(P<0.05).No obvious difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with Henoch-Schönlein purpura nephritis,Tripterysium Glycosides Tablets combined with Qingre Zhixue Decoction can safely and effectively reduce proteinuria level and improve immune status.
