关于CEP72 rs924607 TT基因型和长春碱类相关外周神经毒性的meta分析

摘　　要：目的评价CEP72 rs924607 TT基因型对急性淋巴细胞白血病(ALL)患儿长春碱类药物相关外周神经毒性发生的影响。方法在Pubmed、Embase、Cochrane、中国知网、万方、维普、中国生物医学文献数据库中检索“acute lymphoblastic leukemia”,“vincristine”,“vindesine”,“neuropathy”或其对应的中文词目为检索词,收集2000年1月至2020年1月有关探究CEP72 rs924607遗传变异和发生长春碱类相关外周神经症状之间关系的对比性研究,应用Review Manage5.3软件进行meta分析。结果本研究共纳入相关文献6篇(7个中心研究),得出CEP72 rs924607 TT基因型和长春碱类相关外周神经症状的发生具有明显相关性(OR=2.97,95%CI 1.97-4.47,P<0.00001)。亚组分析发现,在维持阶段儿童和成人ALL患者中,CEP72 rs924607 TT基因型和长春碱类相关外周神经毒性的发生均有关(OR=2.87,95%CI 1.67-4.94,P=0.004)。值得注意的是,在前瞻性研究设计中CEP72 rs924607 TT基因型和外周神经毒性的发生显著相关(OR=4.59,95%CI 2.63-8.01,P<0.00001),而回顾性研究中则显示两者之间差异无显著性(OR=1.46,95%CI 0.85-2.52,P=0.17)。结论CEP72 rs924607 TT基因型是发生长春碱类相关外周神经毒性的危险因素,鉴于纳入研究的固有局限性,需要更多的随机对照试验来确认本研究的结果。Objective To assess the relationship between CEP72 rs924607 TT genotype and vinca alkaloids-induced peripheral neuropathy.Methods By searching the Pubmed,Embase,Cochrane Library,CNKI,Wanfang,Vip and CBM for clinical trials were used to analyze the results for addressing CEP72 rs924607 TT genotype and vinca alkaloids-induced peripheral neuropathy from January 2000 to January 2020.RevMan 5.3 was used for the statistical analysis.Results A total of 6 articles(7 central studies)were included in the study.Meta-analysis showed that CEP72 rs924607 TT genotype was significantly correlated with the occurrence of vinca alkaloids-related peripheral neuropathy(OR=2.97,95%CI 1.97-4.47,P<0.00001).The subgroup analysis found that CEP72 rs924607 TT genotype was associated with the occurrence of vinca alkaloids-related peripheral neurotoxicity during continuation phase in both of adult and pediatric ALL patients(OR=2.87,95%CI 1.67-4.94,P=0.004).It is worth noting that the CEP72 rs924607 TT genotype was significantly associated with peripheral neurotoxicity in the prospective study design(OR=4.59,95%CI 2.63-8.01,P<0.00001).However,there was no statistical difference in retrospective studies(OR=1.46,95%CI 0.85-2.52,P=0.17).Conclusions Current study indicates that the CEP72 rs924607 TT genotype is a risk factor for the occurrence of vinca alkaloids-induced peripheral neuropathy.Given intrinsic restrictions of included studies,more RCTs are warranted to confirm and update the findings of this study.
