温针灸对胃癌手术后化疗患者NK细胞数量及其生物学活性的影响

摘　　要：目的验证温针灸促进胃癌手术后化疗患者免疫功能恢复的效果。方法选取淄博市中心医院肿瘤科2016年10月至2017年10月收治的胃癌患者160例,并将患者按是否接受温针灸分为实验组(80例)与对照组(80例)。两组均给予5-氟尿嘧啶加顺铂化疗方案,实验组于每次化疗后第一天行温针灸治疗,穴取双侧足三里与气海穴,每日1次,连续治疗10天。观察患者外周血象变化,流式细胞术检测自然杀伤细胞(natural killer cell,NK)的变化。结果实验组温针灸治疗后外周血白细胞(white blood cell,WBC)、中性粒细胞(neutrophils,N)、血小板计数(platelet,PLT)及血红蛋白(hemoglobin,Hb)与化疗前相比差异无统计学意义(P>0.05),但对照组治疗后,WBC、N、PLT、Hb的表达水平均较化疗前明显降低[(4.86±1.55)×10^(9)/L比(5.60±1.62)×10^(9)/L;(0.57±0.21)×10^(9)/L比(0.69±0.30)×10^(9)/L;(179.00±47.60)×10^(9)/L比(194.00±55.80)×10^(9)/L;(102.00±17.50)g/L比(113.00±16.90)g/L;t值分别为2.572、2.337、2.134和2.623,P值均<0.05)],差异具有统计学意义。与对照组相比,实验组温针灸治疗后NK细胞数量、NK细胞表达γ-干扰素(interferon-γ,IFN-γ)水平及表达NK细胞活化型受体(natural-killer group 2,member D,NKG2D)水平皆升高[(8.40±2.29)%比(11.86±3.29)%,(9.23±2.51)%比(17.50±4.97)%,(7.50±3.59)%比(11.38±3.16)%,t值分别为2.441、2.355和2.293,P值均<0.05)],差异具有统计学意义。结论温针灸对恢复胃癌术后化疗患者骨髓抑制情况有较好的疗效,并可通过调节NK细胞活性,从而提高机体免疫功能。Objective To verify the effect of warm acupuncture and moxibustion on the recovery of immune function in patients with gastric cancer undergoing chemotherapy after operation.Methods A total of 160 patients with gastric cancer in the Department of Oncology,Zibo Central Hospital from October 2016 to October 2017 were selected and divided into experimental group(n=80)and control group(n=80)according to whether they received warm acupuncture or not.Both groups were given 5-fluorouracil plus cisplatin chemotherapy.The experimental group was treated with warm acupuncture on the first day after each chemotherapy period,and bilateral Zusanli and Qihai acupoints were selected once a day for 10 days.The changes of peripheral hemogram and natural killer cells(NK)were observed using flow cytometry.Results There was no significant difference in peripheral blood white blood cell(WBC),neutrophil(N),platelet(PLT)and hemoglobin(Hb)after warm acupuncture treatment in the experimental group compared with those before chemotherapy(P>0.05).While WBC,N,PLT and Hb in the control group were significantly lower than those before chemotherapy[(4.86±1.55)×10^(9)/L vs(5.60±1.62)×10^(9)/L,(0.57±0.21)×10^(9)/L vs(0.69±0.30)×10^(9)/L,(179.00±47.6)×10^(9)/L vs(194.00±55.80)×10^(9)/L,(102.00±17.50)g/L vs(113.00±16.90)g/L,t values were 2.572,2.337,2.134 and 2.623,respectively,all P values<0.05)].Compared with the control group,the number of NK cells,the expression of interferon-γ(IFN-γ)on NK cells and the expression of NK group-2,member D(NKG2D)in the experimental group were significantly higher than those in the control group[(8.40±2.29)%vs(11.86±3.29)%,(9.23±2.51)%vs(17.50±4.97)%,(7.50±3.59)%vs(11.38±3.16)%,respectively,t values were 2.441,2.355 and 2.293,respectively,all P values<0.05)].Conclusion Warm acupuncture has a good effect on restoring myelosuppression in postoperative chemotherapy patients with gastric cancer,and can improve the immune function by regulating the activity of NK cells.
