摘　　要：目的:总结血栓弹力图(TEG)在使用低分子肝素治疗孕产妇围生期高凝状态时对凝血功能的监测作用。方法:回顾性分析江苏省常州市武进人民医院2017年1月至2019年12月行剖宫产并接受低分子肝素治疗的40例产妇的临床资料,监测用药前、用药后产妇的凝血功能和TEG变化。结果:40例产妇在剖宫产术后使用低分子肝素前和使用3天后的凝血功能各项指标无明显差异(P>0.05),且在正常参考值范围内。40例产妇在剖宫产术后使用低分子肝素前的TEG指标提示为高凝状态[凝血反应时间(R值)为(4.63±0.41)s];在使用低分子肝素3天后,血栓弹力图各项指标趋于正常参考值范围[R值明显升高至(6.70±0.47)s](P<0.05)。结论:血栓弹力图比常规凝血功能检测能更灵敏地反映孕产妇的凝血状态,更利于有效指导围生期的低分子肝素应用。Objective:To summarize the guiding significance of thromboelastography(TEG)by low molecular weight heparin(LMWH)in the treatment of perinatal hypercoagulability.Methods:Clinical data of 40 parturients received LMWH after cesarean section from Jan 2017 to Dec 2019 in Wujin People.s Hospital were analyzed retrospectively.The coagulation and TEG were monitored before and after LMWH treatment.Results:The coagulation function of 40 cases were within the normal reference range before and after LMWH treatment,and there was no statistical difference(P>0.05).The TEG of 40 cases before medication with LMWH indicated hypercoagulability[coagulation reaction time:(4.63±0.41)s].The TEG were normal[coagulation reaction time:(6.70±0.47)s]after LMWH treatment.There was significant statistical difference in the data of ETG before and after LMWH treatment(P<0.05).Conclusions:Comparing with conventional coagulation function test,TEG can reflect the coagulation function of perinatal period and guide the medication.
