开放式乳突根治术联合乳突腔填充治疗胆脂瘤中耳炎的临床可行性

摘　　要：目的分析开放式乳突根治术联合乳突腔填充治疗胆脂瘤中耳炎的临床可行性。方法将本院收治的100例胆脂瘤中耳炎患者随机分为观察组和对照组,各50例。两组均接受开放式乳突根治术治疗,观察组在此基础上联合乳突腔填充治疗。比较两组的治疗效果。结果观察组的术后干耳时间、术腔上皮化时间、切口愈合时间、住院时间均短于对照组(P<0.05)。观察组的外耳道形态正常率及鼓膜愈合率均高于对照组(P<0.05)。观察组治疗后的气导听力、骨导听力、气骨导差及CSOM-QOL各项评分均优于对照组(P<0.05)。结论开放式乳突根治术联合乳突腔填充治疗胆脂瘤中耳炎的效果显著,可改善患者听力水平及外耳道形态,提高鼓膜愈合率及生活质量,值得临床应用。Objective To analyze the clinical feasibility of open radical mastoidectomy combined with mastoid cavity filling in the treatment of cholesteatoma otitis media.Methods A total of 100 cases of cholesteatoma otitis media patients treated in our hospital were randomly divided into observation group and control group,with 50 cases in each group.Both groups were treated with open radical mastoidectomy,and the observation group was treated with mastoid cavity filling on this basis.The therapeutic effects of the two groups were compared.Results The postoperative dry ear time,epithelization of operative cavity time,incision healing time and hospital stay in the observation group were shorter than those in the control group(P<0.05).The normal rate of external auditory canal morphology and tympanic membrane healing rate of the observation group were higher than those of the control group(P<0.05).The air conduction hearing,bone conduction hearing,air-bone gap and CSOM-QOL scores of the observation group after treatment were better than those of the control group(P<0.05).Conclusion The open radical mastoidectomy combined with mastoid cavity filling has a significant effect on the treatment of cholesteatoma otitis media,it can improve the hearing level,the external auditory canal morphology,the tympanic membrane healing rate and quality of life,which is worthy of clinical application.
