加味四黄汤内服外敷对老年混合痔术后患者康复及炎性因子水平的影响

摘　　要：目的观察加味四黄汤内服外敷对老年混合痔术后患者康复及炎性因子水平的影响。方法将南阳市第一人民医院2018年6月至2020年6月收治的100例老年混合痔术后患者随机分为中药治疗组(术后用加味四黄汤内服和外敷治疗)与常规治疗组(术后给予高锰酸钾外用治疗),每组50例。比较两组患者的伤口恢复情况、疼痛情况及炎性因子水平。结果中药治疗组的创面水肿消失时间、渗出物消失时间以及创面愈合时间均短于常规治疗组(P<0.05)。术后3、7 d,中药治疗组的VAS评分均低于常规治疗组(P<0.05)。术后1周,中药治疗组的IL-6、TNF-α、CRP水平均低于常规治疗组(P<0.05)。结论加味四黄汤可明显改善老年混合痔术后患者的伤口恢复情况,减轻术后疼痛程度,还可明显抑制炎性因子的分泌。Objective To observe the effects of internal and external application of Jiawei Sihuang decoction on rehabilitation and inflammatory factors levels in elderly patients with mixed hemorrhoids after operation.Methods A total of 100 elderly patients with mixed hemorrhoids after operation admitted in the Nanyang first people's hospital from June 2018 to June 2020 were randomly divided into traditional Chinese medicine treatment group(treated with internal and external application of Jiawei Sihuang decoction after operation)and conventional treatment group(treated with potassium permanganate externally after operation),with 50 cases in each group.The wound recovery,pain and inflammatory factors levels were compared between the two groups.Results The disappearance time of wound edema,exudate and wound healing time of the traditional Chinese medicine treatment group were shorter than those of the conventional treatment group(P<0.05).The VAS score of the traditional Chinese medicine treatment group were lower than that of the conventional treatment group at 3 and 7 d after operation(P<0.05).The levels of IL-6,TNF-αand CRP of the traditional Chinese medicine treatment group were lower than those of the conventional treatment group at 1 week after operation(P<0.05).Conclusion The Jiawei Sihuang decoction can significantly improve the wound recovery of elderly patients with mixed hemorrhoids after operation,reduce the severity of postoperative pain,and significantly inhibit the secretion of inflammatory factors.
