芪苈强心胶囊联合穴位贴敷对肺心病全心衰患者心肺功能、尿量及NT-proBNP的影响

摘　　要：目的观察芪苈强心胶囊联合穴位贴敷对慢性肺源性心脏病全心心力衰竭(肺心病全心衰)患者心肺功能、尿量及N-末端脑钠肽前体(NT-proBNP)的影响。方法将我院126例肺心病全心衰患者随机分为观察组与对照组各63例。对照组患者采用西医常规治疗,观察组患者在西医常规治疗的基础上联合芪茂强心胶囊、穴位贴敷进行治疗,比较两组临床疗效、心肺功能、尿量和NT-proBNP的水平。结果观察组总有效率显著高于对照组(P<0.05);治疗后两组左心室射血分数[LVEF)、心排出量(CO)及每搏量(SV)、第1秒用力呼气容积占预计值百分比(FEV1/预计值)、1秒通气率[FEV1/FVC)均增高且观察组LVEF、CO、SV、FEV1/预计值、FEV1/FVC均高于对照组(P<0.05);两组24h尿量增高、NT-proBNP水平下降[P<0.05);且观察组24h尿量高于对照组,观察组NT-proBNP水平低于对照组(P<0.05)。结论芪苈强心胶囊联合穴位贴敷对肺心病全心衰患者的疗效较好,明显改善心、肺功能,增加24h尿量,降低NT-proBNP的水平。Objective To observe the clinical effect of Qili Qiangxin Capsule combined with acupoint application on cardiopulmonary function,urine output and N-terminal brain natriuretic peptide precursor(NT-proBNP)in patients with chronic pulmonary heart disease and heart failure due to pulmonary heart disease.Methods 126 patients with pulmonary heart disease and heart failure were randomly divided into observation group and control group,63 cases each.Control group was treated with conventional western medicine;observation group was treated with Qili Qiangxin Capsule combined with acupoint application on the basis of control group.The efficacy cardiopulmonary function,urine output and N-terminal brain natriuretic peptide(NT-proBNP)were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(P<0.05);after treatment,left ventricular ejection fraction(LVEF),cardiac output(CO)and stroke volume(SV)forced air volume in 1 second as a percentage of predicted value(FEV 1/predicted value),1 second ventilation rate(FEV1/FVC)in both groups increased,and LVEF,CO,SV,FEV1/expected value,FEV1/FVC in observation group were higher than those of control group(P<0.05),the 24 h urine volume of both groups increased,NT-proBNP level decreased(P<0.05);the NTproBNP level in observation group was lower than that in control group(P<0.05).Conclusion Qili Qiangxin Capsule combined with acupoint application has better effect on patients with heart failure due to pulmonary heart disease.It obviously improves heart and lung function increases 24 h urine output,and reduces the level of NT-proBNP.
