Solis与Zero-p椎间融合器辅助治疗双节段跳跃型颈椎病的疗效分析

摘　　要：目的比较双节段跳跃型颈椎病患者颈前路减压融合术中应用Solis、Zero-p椎间融合器辅助治疗的有效性及安全性。方法双节段跳跃型颈椎病患者45例,行Solis椎间融合器辅助的颈前路减压融合术25例为Solis组,行Zero-p椎间融合器辅助的颈前路减压融合术20例为Zero-p组。比较2组手术时间、术中出血量、住院时间及术后并发症发生情况;随访至2020年6月,比较术前、术后3个月、末次随访时日本骨科学会(Japanese Orthopaedic Association,JOA)评分、颈椎功能障碍指数(neck disability index,NDI)、视觉模拟评分(visual analogue scale,VAS);比较术前、术后3个月、术后12个月、末次随访时颈椎曲度、跳跃节段活动度、融合节段椎间隙高度;比较术后3、6、12个月融合节段融合率;比较末次随访时邻近节段、跳跃节段退变率。结果Solis组手术时间[(96.20±14.38)min]短于Zero-p组[(108.00±14.99)min](P<0.05),术中出血量[(76.28±13.86)mL]少于Zero-p组[(85.45±13.81)mL](P<0.05);2组住院时间比较差异无统计学意义(P>0.05)。2组术后3个月、末次随访时JOA评分均高于术前,NDI评分、VAS评分均低于术前(P<0.05);2组末次随访时JOA评分高于术后3个月,NDI评分低于术后3个月(P<0.05),VAS评分与术后3个月比较差异无统计学意义(P>0.05)。Solis组与Zero-p组术前、术后3个月、末次随访时JOA评分、NDI评分、VAS评分比较差异均无统计学意义(P>0.05)。2组术后3、12个月、末次随访时颈椎曲度及融合节段椎间隙高度均大于术前(P<0.05),2组术后3个月融合节段椎间隙高度大于术后12个月及末次随访时(P<0.05);2组术后3、12个月及末次随访时跳跃节段活动度与术前比较差异均无统计学意义(P>0.05)。Solis组与Zero-p组术前、术后3个月、术后12个月、末次随访时颈椎曲度、跳跃节段活动度、融合节段椎间隙高度比较差异均无统计学意义(P>0.05)。Solis组术后3、6个月融合率(76Objective To investigate the efficacies and safeties of Solis and Zero-p implant systems in anterior cervical disectomy and fusion for two-segment skip cervical spondylosis.Methods Forty-five patients with two-segment skip cervical spondylosis were randomly divided into 25patients receiving anterior cervical discectomy and fusion with Solis implant system(Solis group)and 20patients receiving anterior cervical discectomy and fusion with Zero-p implant system(Zero-p group).The operation time,intraoperative blood loss,length of hospital stay and complications were compared between two groups.All patients were followed up till June,2020.Japanese Orthopedic Association(JOA)score,neck disability index(NDI)and visual analogue scale(VAS)score were compared before operation,3months after operation and at the final follow-up.The cervical curvature,the range of motion of skip-level segment and the disc height of operated segments were compared before operation,3months after operation,12months after operation and at the final follow-up,the bone fusion rate was compared 3,6and 12months after operation,and the adjacent and skip-level segment degeneration rates were compared at the final follow-up between two groups.Results The operation time was shorter and intraoperative blood loss was less in Solis group((96.20±14.38)min,(76.28±13.86)mL)than that in Zero-p group((108.00±14.99)min,(85.45±13.81)mL)(P<0.05),and the length of hospital stay showed no significant difference between two groups(P>0.05).The JOA score was higher 3months after operation and at the final follow-up than that before operation(P<0.05),while NDI and VAS score were lower than those before operation(P<0.05).The JOA score was higher and NDI was lower at the final follow-up than that 3months after operation(P<0.05),and VAS score showed no significant differences at the final follow-up compared with that 3months after operation(P>0.05).The JOA,NDI and VAS score before operation,3months after operation and at the final operation showed no significant differen
