3~15岁儿童变应性鼻炎舌下免疫治疗的短期疗效

摘　　要：目的:探讨儿童变应性鼻炎进行舌下特异性免疫治疗的短期疗效。方法:采用自身前后对照,比较71例3~15岁粉尘螨致敏的变应性鼻炎儿童在进行标准化的粉尘螨滴剂舌下特异性免疫治疗后3、6、9、12个月的疗效情况。疗效评估包括鼻炎症状总评分、药物总评分和视觉模拟量表(visual analogue score,VAS)评分情况。结果:进行粉尘螨舌下特异性免疫治疗后的3、6、9、12个月时患者的鼻炎症状总评分、药物总评分以及VAS评分与治疗前相比均有下降,差异有统计学意义(P <0.05)。6个月与3个月相比,以及9个月与6个月相比,鼻炎症状总评分、药物总评分和VAS评分均有下降,差异有统计学意义(P <0.05),12个月与9个月相比药物总评分无统计学差异(P> 0.05),但鼻炎症状总评分和VAS评分仍均有下降,差异有统计学意义(P <0.05)。结论:粉尘螨舌下特异性免疫治疗对儿童变应性鼻炎的短期疗效良好,在3个月就能起效,并在6、9、12个月时得到持续改进。Objective:This study aims to explore the short-term effect of sublingual allergen-specific immunotherapy in children with allergic rhinitis. Methods:There were 71 subjects of 3-15 years old with allergic rhinitis who were sensitized to dermatophagoides farinae. They were treated with standardized sublingual dermatophagoides farinae immunotherapy drops. Then the short-term effects of the treatment were evaluated by comparing the outcome scores including the total rhinitis symptom scores,total medication scores and visual analogue scale(VAS)before and after the treatment for every three months until a year. Results:Total rhinitis symptom scores,total medication scores and VAS were all declined in 3 months,6 months,9 months and 12 months after the treatment compared with those before the treatment. Meanwhile,the three scores were all declined in 6 months and 9 months compared with those in 3 months and 6 months,respectively. Total rhinitis symptom scores and VAS were both declined while no difference was shown in total medication scores in 12 months compared with those in 9 months. Conclusion:In the short term,sublingual dermatophagoides farinae immunotherapy is an effective way to treat 3-15 years old children with allergic rhinitis who are sensitized to dermatophagoides farina,which can take effect in 3 months after the treatment,and becomes more and more effective in 6 month,9 months and 12 months.
