土茯苓汤联合复方青黛膏治疗寻常型银屑病临床研究

摘　　要：目的:观察土茯苓汤联合复方青黛膏治疗寻常型银屑病的临床疗效。方法:将116例寻常型银屑病患者按随机数字表法分为治疗组和对照组,每组53例。治疗组给予土茯苓汤联合复方青黛膏治疗,对照组给予阿维A胶囊联合卡泊三醇软膏治疗,2组均治疗2个月。比较2组临床疗效及治疗前后银屑病面积和严重程度指数(PASI)、血清炎症因子水平。结果:治疗后,2组PASI评分均较治疗前降低(P<0.01),治疗组PASI评分较对照组下降更明显(P<0.01)。治疗组愈显率为90.6%,对照组愈显率为58.5%,2组比较,差异有统计学意义(P<0.01)。治疗后,2组血清血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)、白细胞介素-17 (IL-17)、白细胞介素-22 (IL-22)水平均较治疗前降低(P<0.01),治疗组血清VEGF、TNF-α、CRP、IL-17、IL-22水平均低于对照组(P<0.01)。结论:土茯苓汤联合复方青黛膏治疗寻常型银屑病疗效确切,其作用机制可能与降低血清VEGF、TNF-α、CRP、IL-17、IL-22水平有关。Objective: To observe the clinical effect of Tufuling tang combined with compound Qingdai ointment for psoriasis vulgaris. Methods:A total of 116 cases of patients with psoriasis vulgaris were divided into the treatment group and the control group according to the random number table method,53 cases in each group. The treatment group was treated with Tufuling tang combined with compound Qingdai ointment, and the control group was treated with avi A capsules combined with calcipotriol ointment. Both groups were treated for two months. The levels of Psoriasis Area and Severity Index(PASI) and serum inflammatory factors before and after treatment as well as the clinical effect in the two groups were compared. Results:After treatment,the PASI scores in the two groups were decreased when compared with those before treatment(P<0.01),and the decrease in the treatment group was more obvious than that in the control group(P<0.01). The cure and effective rate was 90.6% in the treatment group and 58.5% in the control group,the difference being significant(P<0.01). After treatment,the levels of vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-17(IL-17) and interleukin-22(IL-22) in serum in the two groups were decreased when compared with those before treatment(P<0.01);the levels of VEGF,TNF-α,CRP,IL-17 and IL-22 in serum in the treatment group were lower than those in the control group(P<0.01). Conclusion: The therapy of Tufuling tang combined with compound Qingdai ointment has definite curative effect in treating psoriasis vulgaris,whose mechanism may be related to the reduction of the levels of VEGF,TNF-α,CRP,IL-17 and IL-22 in serum.
