从三才思想探析针刺复式补泻手法透天凉

摘　　要：透天凉手法作为传统针刺复式补泻手法的代表,临床疗效明确;其手法以呼吸、提插、捻转和徐疾等单式手法相合而成,引阳外出而达到泻热倒阴之效,初学之人难以掌握操作和临床运用,其机理理论探讨有待完善。尽管现代针灸名家都在手法基础上继承和创新,但目前后学之人仍难以领会;以三才思想对透天凉机理提出一些见解和补充,认为透天凉病机重在卫不在其位,治则目的则是卫归其位,引卫外出是产生凉感的关键,这对透天凉手法理论丰富和传承有一定的指导意义。Cool-producing needling,as a representative of traditional acupuncture manipulation of compound reinforcing-reducing,the therapeutic effect of the manipulation is significant.It is composed of single techniques including inhaling and exhaling technique,lifting and inserting technique,twirling technique and slow-and quick technique.The manipulation can induce yang to reach out to achieve the effect of reducing heat,which is difficult for beginners to master the operation and clinical application,and its mechanism theory needs to be improved.Although modern acupuncturists inherit and innovate the basis of techniques,it is still hard for later scholars to understand.This study put forward some opinions and supplements of techniques of cool-producing needling based on thinking of the three types of talent.It is believed that its pathogenesis is mainly defensive-qi not in the position and the aim of its treatment is to return defensive-qi to the proper place,and leading defensive-qi out is the key to produce cool feeling,which has a certain guiding significance for the enrichment and inheritance of cool-producing needling.
