摘 要:目的:观察电针对2型糖尿病胰岛素抵抗大鼠血糖、血脂和肝脏相关酶学指标以及肝细胞中AMPK、GLUT4表达的影响,探讨电针调节大鼠肝脏细胞缓解2型糖尿病胰岛素抵抗的作用机理。方法:选用16只同期且体质量2月龄的SPF级雄性ZDF(Zucker Diabetic Fatty rat)大鼠(fa/fa)作为造模组及8只同期且体质量2月龄的SPF级雄性ZL(Zuker lean)对照大鼠(fa/+)作为空白组,待16只大鼠造模成功后随机区分为两组:模型组、电针组。干预周期为4周,每周6 d。于每周六早8:00对各组大鼠采取尾部静脉血,检测各组大鼠空腹血糖。末次干预后取各组大鼠血清检测空腹血糖(FBG)、胰岛素(INS)、C-肽(C-P)水平、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、极低密度脂蛋白(vLDL)、甘油三酯(TG)及总胆固醇(TC)等生化指标,计算OGTT曲线下面积,测定肝脏相关谷丙转氨酶(ALT)、谷草转氨酶(AST)和γ-谷氨酰转肽酶(GGT)酶学指标水平,取部分肝脏组织运用western blot法检测腺苷酸活化蛋白激酶(AMPK)、葡萄糖转运蛋白4(GLUT4)的蛋白表达含量,比较各组大鼠血脂、血糖、胰岛素水平、肝细胞中转氨酶及AMPK、GLUT4等相关指标的差异变化。结果:干预4周后,与空白组比较,模型组各项血糖、血脂、转氨酶中ALT和GGT水平均显著升高(P<0.05),AST水平亦升高,但差异无统计学意义(P>0.05);与模型组相比,电针组血糖、血脂中各项指标水平均显著下降(P<0.05),血脂指标TG水平差异无统计学意义(P>0.05);与模型组相比,电针组转氨酶指标GGT水平显著下降,差异有统计学意义(P<0.05),AST、ALT虽水平下降,但差异无统计学意义(P>0.05);与空白组比较,模型组AMPK、GLUT4含量显著下降,差异有统计学意义(P<0.01),与模型组相比,电针组AMPK、GLUT4含量表达明显上升,差异有统计学意义(P<0.01)。结论:电针可以降低T2DM胰岛素抵抗大鼠血糖、血清血脂和肝细胞相关转氨酶水平,调节肝Objective:To observe the effects of electro-acupuncture on blood glucose,blood lipid,liver serum enzyme indexes and the expressions of AMPK and GLUT4 in insulin resistant rats with type 2 diabetes mellitus(T2DM),and to explore the mechanism of electro-acupuncture regulating rat liver cells to alleviate insulin resistance(IR)of T2DM.Methods:16 SPF male ZDF(Zucker Diabetic Fatty rat)rats(fa/fa)with the same period and the body weight of 2 months were selected into the modeling group and 8 SPF male ZL(Zuker lean)control rats(fa/+)with the same period and the body weight of 2 months were selected as the blank control.After successful modeling,16 rats were randomly divided into the model group and the electro-acupuncture group.The intervention was 6 days per week for 4 weeks.Blood was taken from the tail at 8:00 a.m.every Saturday,and the fasting blood glucose(FBG)was measured.After the last intervention,serum was taken from each group to determine FBG,serum insulin(INS)and C-peptide levels(C-P),high-density lipoprotein(HDL),low-density lipoprotein(LDL),very low density lipoprotein(vLDL),triglyceride(TG),total cholesterol(TC)and other biochemical indicators;the OGTT area under the curve was calculated;enzyme index levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and gamma-glutamyl transpeptidase(GGT)were detected;the Western blot method was used to detect the protein expressions of AMPK and GLUT4 of liver tissue.The level changes of blood lipid,blood glucose,insulin,transaminase,AMPK and GLUT4 were compared in each group.Results:After 4 weeks of intervention,the levels of blood glucose,blood lipids,ALT and GGT were significantly increased in the model group compared to those in the blank group(P<0.05);the level of AST was increased,but there was no statistical difference between the model group and the blank group(P>0.05).Compared to those in the model group,the levels of blood glucose and blood lipid were significantly decreased in the electro-acupuncture group(P<0.05);there was no statistical d
