中药温阳化饮方治疗慢性肺源性心脏病的临床观察

摘　　要：探讨中药温阳化饮方治疗慢性肺心病对患者肺功能和生活质量的影响。将2018年10月—2019年12月保定市第四中心医院收治的90例慢性肺心病患者按随机抽签法分为对照组和观察组,每组均45例。对照组给予常规对症治疗,观察组联用中药温阳化饮方治疗并随症加减,治疗14 d后评估疗效。比较2组治疗前后证候积分、肺功能指标、圣乔治呼吸疾病问卷(SGRQ)评分和不良反应发生率。研究结果显示:治疗前2组证候积分差异均无统计学意义(P>0.05),治疗后上述积分均较治疗前下降(P<0.05),观察组证候积分低于对照组(P<0.05);治疗前2组第1秒用力呼气量(FEV 1)、第1秒用力呼气量与用力肺活量比值(FEV 1/FVC)相当(P>0.05),治疗后均明显升高(P<0.05),观察组优于对照组(P<0.05);治疗前2组SGRQ各维度及总评分差异无统计学意义(P>0.05),治疗后2组评分均明显下降(P<0.05),观察组低于对照组(P<0.05);2组均未见药物相关不良反应。研究认为温阳化饮方治疗慢性肺心病能有效缓解症状,改善肺功能和生活质量,安全性较高,整体疗效显著。The purpose of this paper is to explore the effect of Wenyang Huayin Decoction on the lung function and quality of life of patients with chronic pulmonary heart disease.90 patients with chronic pulmonary heart disease admitted to our hospital from October 2018 to December 2019 were randomly divided into control group and observation group,with 45 cases in each.The control group was given conventional symptomatic treatment,and the observation group was treated with traditional Chinese medicine Wenyang Huayin Decoction,which was added or subtracted according to the symptoms.The curative effect was evaluated 14 days after treatment.The symptom scores,pulmonary function indexes,St.George's Respiratory Disease Questionnaire(SGRQ)score and the incidence of adverse reactions were compared between the two groups before and after treatment.The results of the study showed that there was no significant difference in the syndrome scores between the two groups before treatment(P>0.05).After treatment,the above scores were all lower than these before treatment(P<0.05).The observation group's syndrome scores were lower than those of the control group(P<0.05).Before treatment,the forced expiratory volume(FEV 1)in the first second and the ratio of the forced expiratory volume in the first second to the forced vital capacity(FEV 1/FVC)of the two groups were similar(P>0.05),and both increased significantly after treatment(P<0.05).The observation group is better than the control group(P<0.05).There was no statistically significant difference in SGRQ dimensions and total scores between the two groups before treatment(P>0.05).After treatment,the scores of the two groups were significantly reduced(P<0.05),and the observation group was lower than the control group(P<0.05).There were no drug-related adverse reactions in both groups.The study believes that Wenyang Huayin therapy can effectively relieve symptoms,improve lung function and quality of life in the treatment of chronic pulmonary heart disease,with high safety and significant ov
