健脾补肾化瘀汤联合免疫抑制治疗慢性再生障碍性贫血的疗效研究

摘　　要：为观察健脾补肾化瘀汤联合免疫抑制疗法治疗慢性再生障碍性贫血的临床疗效,选取入平凉市第二人民医院血液肿瘤科治疗的慢性再生障碍性贫血的患者64例,随机分组后对照组32例单纯接受免疫抑制疗法治疗(口服环孢素A,3~5 mg/(kg·d),分2次口服;口服司坦唑醇片,3次/d,2 mg/次),观察组32例在上述处理基础上联合健脾补肾化瘀汤治疗。两组均治疗2个疗程后评估疗效。结果显示:治疗后观察组临床疗效(临床治愈率+缓解率+好转率)优于对照组(P<0.05),中医证候(主次症)积分低于对照组(P<0.05);治疗后两组外周血象均明显改善,观察组改善幅度高于对照组(P<0.05);治疗后两组血栓弹力图血小板最大血块强度(Ma)值均较治疗前升高(P<0.05),观察组升高幅度高于对照组(P<0.05);两组均无严重不良反应。研究表明健脾补肾化瘀汤联合免疫抑制疗法治疗慢性再生障碍性贫血能提高疗效,改善外周血象和凝血功能。The purpose of this paper is to observe the clinical efficacy of Jianpi Bushen Huayu Decoction combined with immunosuppressive therapy in the treatment of chronic aplastic anemia.64 patients with chronic aplastic anemia who were admitted to the department of hematology and oncology in Pingliang Second People's Hospital were selected and were divided into two groups according to different treatment plans.32 cases in the control group received only immunosuppressive therapy(oral cyclosporine A,2 times/d,3~5 mg/(kg·d),oral stanozolol tablets,3 times/d,2 mg each time),32 cases in the observation group on the basis of the above treatment,combined with Jianpi Bushen Huayu Decoction.The curative effect was evaluated after 2 courses of treatment in both groups.After treatment,the clinical efficacy of the observation group(clinical cure rate+remission rate+improvement rate)was better than that of the control group(P<0.05),and the scores of TCM syndromes(primary and secondary symptoms)were lower than those of the control group(P<0.05).After treatment,the peripheral hemogram of the two groups was significantly improved,and the improvement range of the observation group was higher than that of the control group(P<0.05).After treatment,the maximum platelet amplitude(Ma)value of the thromboelastogram in the two groups was higher than that before treatment(P<0.05),and the increase in the observation group was higher than that in the control group(P<0.05).There were no serious adverse reactions in both groups.The results show that Jianpi Bushen Huayu Decoction combined with immunosuppressive therapy can improve the curative effect of chronic aplastic anemia,peripheral blood picture and blood coagulation function.
