摘　　要：目的无论平时战时,创伤性颅脑伤发生率高,伤情复杂、发展快、危害大,旨在探讨如何将重型颅脑伤患者通过直升机快速、安全后送至具有救治条件的医院。方法分析解放军联勤保障部队第九四〇医院利用直升机空运后送4例重型颅脑伤患者救治过程并回顾总结相关研究进展。结果4例病情极危重,合并多发伤,其中2例无自主呼吸,所在医院位置偏远,不具备进一步救治条件,通过直升机空运后送可以缩短转运时间,且机载呼吸机及监护设备可以监测及维持患者的生命体征,可以将患者快速、安全转运至我院接受进一步治疗。结论转运前的系统性评估、建立稳定的气道和可靠的静脉通道,转运途中对生命体征监测及维持是转运成功的关键;多次空运后送实践提示完善空运后送流程,优化空运后送衔接管理,可以使重型颅脑伤患者在空运后送中受益。Objective To explore how to quickly and safely transfer patients with severe craniocerebral injury by helicopter to better-equipped hospitals.Methods The process of treating four patients with severe craniocerebral injur y sent by helicopter to our hospital was analyzed while the progress in related research was reviewed.Results These four patients were in critical condition with multiple injuries,two of whom could not spontaneously breathe.The hospital that admitted them was remotely located and incapable of providing further treatment.Air transportation by helicopter could obviously shorten the time of transfer,and the vital signs of the patients could be monitored and maintained by the ventilator and monitoring equipment onboard,ensuring quick and safe transfer of the patients to our hospital for further treatment.Conclusion The keys to successful transfer include a systematic evaluation before transportation,guarantee of a stable airway and reliable venous channel,as well as the monitoring and maintenance of vital signs during transportation.Besides,multiple cases of aeromedical evacuation indicate that patients with severe craniocerebral injury can benefit from the improvement of the evacuation process and the optimization of the links of the process.
