经尿道前列腺等离子电切术治疗前列腺增生效果分析

摘　　要：目的探讨经尿道前列腺等离子电切术(Transurethral plasma kinetic resection of prostate,PKRP)治疗前列腺增生的临床效果。方法选择2019年1月至2020年1月于本院进行手术治疗的前列腺增生患者113例,分为对照组56例和观察组57例。对照组采用经尿道前列腺电切术(Transurethral resection of prostate,TURP)治疗,观察组采用经尿道前列腺等离子电切术治疗。对比两组患者围术期各项指标,手术前后实验室指标、尿路功能指标和并发症发生情况。结果观察组手术时间、术中出血量、膀胱冲洗时间、导尿管拔除时间和住院时间均低于对照组(P<0.05);观察组出院时,实验室指标血前列腺特异性抗原(PSA)、血小板衍生生长因子BB(PDGF-BB)和单核细胞趋化因子-1(MCP-1)水平,膀胱残余尿量(RU)、国际前列腺症状评分(IPSS)和生活质量评分(QoL)及并发症发生率均低于对照组,最大尿流率(Qmax)高于对照组,差异有统计学意义(P<0.05)。结论PKRP可有效缩短前列腺增生患者手术时间、膀胱冲洗时间等围术期指标,改善PSA等实验室指标和尿路功能,降低并发症发生率,安全性较好。Objective To explore the clinical effect of transurethral plasma kinetic resection of prostate(PKRP)for benign prostatic hyperplasia(BPH).Methods A total of 113 patients with BPH who received surgery treatment in our hospital from January 2019 to January 2020 were selected and divided into the control group(n=56)and the observation group(n=57).The control group was given transurethral resection of prostate(TURP)treatment,and the observation group was given PKRP treatment.The perioperative indexes,laboratory indexes before and after surgery,urinary tract function indexes and incidence of complications were compared between two groups.Results The operation time,intraoperative blood loss,bladder irrigation time,catheter removal time and hospital stay in the observation group were significantly lower than those in the control group(P<0.05).When discharged from the hospital,the laboratory indexes PSA,PDGF-BB and MCP-1 of patients in the observation group were significantly lower than those in the control group(P<0.05);the RU,IPSS score and QoL score and the incidence of complications of the observation group were significantly lower than those of the control group;Qmax in the observation group was significantly higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Transurethral PKRP can effectively shorten the perioperative indicators such as operation time and bladder flushing time of patients with BPH,and can improve the laboratory indicators such as PSA and the urinary tract function,reduce the incidence of complications,and has good safety.
