降糖益肾汤联合前列地尔治疗糖尿病肾病临床研究

摘　　要：目的:观察降糖益肾汤联合前列地尔对糖尿病肾病患者全段成纤维细胞生长因子-23 (iFGF-23)、α-Klotho、尿泛素-核糖体结合蛋白52 (UBA52)的影响。方法:选取糖尿病肾病患者118例作为研究对象,按照随机数表法分为对照组和观察组,每组59例。对照组采用前列地尔治疗,观察组在对照组基础上联合降糖益肾汤治疗。比较2组临床疗效及治疗前后血清iFGF-23、α-Klotho、UBA52、肌酐(Cr)、尿素氮(BUN)、总胆固醇(TC)、甘油三酯(TG)、肌酐清除率(CCr)、尿白蛋白排泄率(UAER)的变化情况。结果:观察组总有效率为94.92%,显著高于对照组的83.05%(P<0.05)。与同组治疗前比较,治疗后2组血清iFGF-23、UBA52、BUN、Cr、TC、TG、UAER均降低(P<0.05),α-Klotho、CCr均升高(P<0.05)。与对照组治疗后比较,观察组治疗后血清iFGF-23、UBA52、BUN、Cr、TC、TG、UAER均降低(P<0.05),α-Klotho、CCr均升高(P<0.05)。结论:降糖益肾汤联合前列地尔可调节糖尿病肾病患者iFGF-23、α-Klotho、UBA52水平,保护肾功能,纠正代谢紊乱,较单独应用前列地尔治疗疗效更佳。Objective:To observe the effect of Jiangtang Yishen tang combined with alprostadil on intact fibroblast growth factor-23(iFGF-23), α-Klotho and ubiquitin-ribosomal binding protein 52(UBA5) in patients with diabetic nephropathy.Methods:A total of 118 cases of patients with diabetic nephropathy were selected as the study objects,and were divided into the control group and the observation group according to the random number table method,59 cases in each group. The control group was treated with alprostadil, and the observation group was additionally treated with Jiangtang Yishen tang based on the treatment of the control group. The changes of serum iFGF-23,α-Klotho,UBA52,creatinine(Cr),blood urea nitrogen(BUN), total cholesterol(TC), triacylglycerol(TG), creatinine clearance rate(CCr), urinary albumin excretion rate(UAER) before and after treatment as well as the clinical effect in the two groups were compared. Results:The total effective rate was 94.92% in the observation group, significantly higher than that of 83.05% in the control group(P<0.05). After treatment,serum iFGF-23,UBA52,BUN,Cr,TC,TG and UAER in the two groups were all decreased when compared with those before treatment(P<0.05),and α-Klotho and CCr were increased(P<0.05). After treatment,serum iFGF-23,UBA52,BUN,Cr,TC,TG and UAER in the observation group were all decreased when compared with those in the control group(P<0.05), and α-Klotho and CCr were increased(P<0.05). Conclusion: The therapy of Jiangtang Yishen tang combined with alprostadil can regulate the levels of i FGF-23, α-Klotho and UBA52 in patients with diabetic nephropathy,protect kidney function,and correct metabolic disorders,whose curative effect is better than that of simple alprostadil.
