滋阴助阳方联合二甲双胍治疗多囊卵巢综合征临床研究

摘　　要：目的:观察滋阴助阳方联合二甲双胍治疗多囊卵巢综合征(PCOS)的临床疗效。方法:选取90例PCOS患者,根据入院编号随机分为观察组和对照组各45例。对照组给予盐酸二甲双胍片治疗,观察组给予滋阴助阳方联合二甲双胍治疗,以1个月经周期为1个疗程,2组均治疗3个疗程。比较2组治疗前后激素水平、子宫内膜厚度及子宫螺旋动脉搏动指数(PI)和阻力指数(RI),比较2组临床妊娠率及生化妊娠率。结果:治疗后,2组促卵泡激素(FSH)、促黄体激素(LH)、睾酮(T)及抗苗勒管激素(AMH)水平均较治疗前降低(P<0.05),观察组FSH、LH、T及AMH水平均低于对照组(P<0.05)。治疗后,2组子宫内膜厚度均较治疗前增加(P<0.05),子宫螺旋动脉PI、RI均较治疗前降低(P<0.05);观察组子宫内膜厚度大于对照组(P<0.05),子宫螺旋动脉PI、RI均低于对照组(P<0.05)。观察组生化妊娠率为26.67%,低于对照组的42.22%;观察组临床妊娠率为55.56%,高于对照组的22.22%,差异均有统计学意义(P<0.05)。结论:滋阴助阳方联合二甲双胍治疗PCOS,可有效调节患者的激素水平,增加其子宫内膜厚度,改善子宫螺旋动脉PI、RI,提高临床妊娠率。Objective:To observe the clinical effect of Ziyin Zhuyang prescription combined with metformin for polycystic ovary syndrome(PCOS). Methods:A total of 90 cases of PCOS patients were selected and divided into the observation group and the control group according to the admission number,45 cases in each group. The control group was given metformin hydrochloride tablets, and the observation group was additionally treated with Ziyin Zhuyang prescription based on the treatment of the control group. Both groups were treated for three courses, one menstrual cycle being a course. The hormone levels,endometrial thickness,as well as pulsatility index(PI) and resistance index(RI) of uterine spiral artery were compared between the two groups before and after treatment. The clinical pregnancy rate and biochemical pregnancy rate in the two groups were compared. Results: After treatment, the levels of follicle stimulating hormone(FSH), luteinizing hormone(LH), testosterone(T) and anti mullerian hormone(AMH) in the two groups were decreased when compared with those before treatment(P<0.05),and the four levels above in the observation group were lower than those in the control group(P<0.05). After treatment,the endometrial thickness of two groups were increased when compared with those before treatment(P<0.05),and PI and RI of uterine spiral artery were decreased when compared with those before treatment(P<0.05);the endometrial thickness in observation group was larger than that in the control group(P<0.05),and PI and RI of uterine spiral artery were lower than those in the control group(P<0.05). The biochemical pregnancy rate was 26.67% in the observation group,lower than that of 42.22% in the control group(P<0.05). The clinical pregnancy rate was 55.56% in the observation group, higher than that of 22.22% in the control group(P<0.05). Conclusion: The therapy of Ziyin Zhuyang prescription combined with metformin in the treatment of PCOS can effectively regulate hormone levels, enhance the endometrial thickness,improve the PI and
