宫外孕Ⅱ号方内服外敷联合常规药物保守治疗异位妊娠临床研究

摘　　要：目的:观察宫外孕Ⅱ号方内服、外敷联合常规药物保守治疗异位妊娠(EP)的临床疗效。方法:将120例患者按随机数字表法分为对照组和观察组各60例。2组均予甲氨蝶呤(MTX) 2次剂量方案,并根据人绒毛膜促性腺激素(HCG)下降速度给药,直至HCG恢复正常,或转手术治疗;米非司酮片使用5 d。观察组加予宫外孕Ⅱ号方内服和外敷,连续用药14 d。记录阴道出血停止时间、腹痛消失时间、HCG恢复正常时间及妊娠包块吸收时间。比较2组的治疗效果。治疗结束后6个月,行子宫输卵管造影检查复通情况。结果:治疗后,观察组治愈率为91.67%,高于对照组的76.67%,差异有统计学意义(P<0.05)。观察组阴道出血停止时间、腹痛消失时间、HCG恢复正常时间及妊娠包块吸收时间均较对照组加快(P<0.01)。治疗结束后6个月,观察组输卵管复通率为85.00%,高于对照组的66.67%,差异有统计学意义(P<0.05)。结论:采用宫外孕Ⅱ号方内服、外敷联合MTX与米非司酮的保守治疗方案近期可缩短EP病程,提高保守治疗的成功率,从远期来看,可促进输卵管复通,从而有利于后期的再生育,临床疗效较为确切。Objective:To observe the clinical effect of internal and external application of self-made ectopic pregnancy No.2 prescription combined with conservative treatment of routine medicine for ectopic pregnancy(EP). Methods:A total of120 cases of patients were divided into the control group and the observation group according to the random number table method,60 cases in each group. Both groups were given two-dose methotrexate(MTX) protocol,and the administration was given according to the descent speed of human chorionic gonadotropin(HCG) until HCG returned to normal or the transfer to operative treatment occurred;mifepristone tablets were applied for five days. The observation group was additionally given internal and external application of self-made ectopic pregnancy No.2 prescription for 14 days. The stopping time of vaginal bleeding, the disappearance time of abdominal pain, the time for HCG to return to normal and the absorption time of pregnancy package were recorded. The clinical effect in the two groups was compared. Six months after treatment, the hysterosalpingography was performed to exam the state of recanalization. Results: After treatment, the cure rate was91.67% in the observation group,higher than that of 76.67% in the control group,the difference being significant(P<0.05).The stopping time of vaginal bleeding,the disappearance time of abdominal pain,the time for HCG to return to normal and the absorption time of pregnancy package in the observation group were accelerated when compared with those in the control group(P<0.01). Six months after treatment, the fallopian tube recanalization rate was 85.00% in the observation group,higher than that of 66.67% in the control group,the difference being significant(P<0.05). Conclusion:In the short term,the application of internal and external application of self-made ectopic pregnancy No.2 prescription combined with conservative treatment of MTX and mifepristone can shorten the EP course,improve the success rate of conservative treatment.
