血清CA125、CA153、HE4在卵巢癌诊断中的价值探讨

摘　　要：目的研究探讨CA125、CA153、HE4三种血清指标检测,在卵巢癌诊断中的价值.方法随机选取我院于2017年6月~2019年6月收治的卵巢肿瘤患者160例,分为卵巢良性病变组(79例)以及卵巢癌组(81例),再随机选取健康体检者60例作为对照组,对所有参与者进行CA125、CA153、HE4血清检测,观察其血清水平.结果三组各肿瘤标志物阳性率比较,卵巢癌组明显高于卵巢良性病变组以及对照组,P<0.05;各肿瘤标志物的肿瘤不同病理分期的血清水平比较,发现卵巢良性病变、卵巢癌(TNM)Ⅰ、Ⅱ期、卵巢癌(TNM)Ⅲ、Ⅳ期的CA153、CA125、HE4水平逐步上升,P<0.05;各肿瘤标志物的诊断效能比较,发现单项检测时,各肿瘤指标有其各自的优缺点,但当联合检测时,其特异性、灵敏度、阳性预测值及阴性预测值均提高,弥补了单项检测的不足之处,P<0.05.结论CA125、CA153、HE4三种血清指标检测在卵巢癌诊断中,各有其优点与不足之处,但当联合检测时,其特异性、灵敏度、阳性预测值及阴性预测值均提高,弥补了单项检测的不足之处,提高了卵巢癌的早期诊断率,值得在临床中推广.Objective To study the value of CA125,CA153 and HE4 in the diagnosis of ovarian cancer.Methods A total of 160 patients with ovarian cancer admitted to our hospital from June 2017 to June 2019 were randomly divided into two groups:the benign ovarian disease group(79 cases)and the ovarian cancer group(81 cases).Then 60 healthy people were randomly selected as the control group.The serum levels of CA125,CA153 and HE4 were measured for all participants.Results The positive rates of tumor markers in the three groups were significantly higher than those in the benign ovarian disease group and the control group(P<0.05).Compared with the diagnostie efficacy of the markers,it was found that each tumor index had its own advantages and disadvantages in single detection,but when combined detection,its specificity,sensitivity,positive predictive value and negative predictive value were all improved,making up for the shortcomings of single detection,P<0.05.Conclusion CA125,CA153 and HE4 have their own advantages and disadvantages in the diagnosis of ovarian cancer,but when combined detection,their speifieity,sensitivity,positive predictive value and negative predictive value are all improved,which make up for the shortcomings of single detection and improve the early diagnosis rate of ovarian cancer,which is worth popularizing in elinic.
